Guidelines for the General Nursing Care Study
Activities
1  Identify a client (Must be picked from Kapenguria County Referral Hospital; must
have a medical or surgical condition, paediatric cases will be considered on a case to
case basis)
 Obtain a comprehensive written/signed consent, (see attached sample) – inform the
client of the intent to visit them at home to assess the environment and interact with
other family members.
 Obtain a comprehensive history (Also include the management of the present illness
with the investigations ordered, drugs used, surgical procedures done etc)
 Conduct a detailed physical examination (Remember to have a focused physical
examination for affected system).
 Generate a Nursing Care Plan with not less than five priority diagnosis.
 Have the ward in-charge countersign to confirm the details of the client (Name, IP
number, Diagnosis)

Present your write up to your supervisor for discussion and review/approval.
2 Conduct a literature review of the condition/s under study, with relevant references and
citations not older than 8 years.
 Anatomy and Physiology relevant to patient’s condition
 Comparison of actual disease and patient’s illness
 Comparison of the actual disease management and ideal management
Conduct a Familiarization visit to the client’s home

 Obtain the following information:
 Adults – Names, Ages, Occupation, Level of Education, State of Health.
 Children – Names, Ages, Immunisation status, Level of Education, State of
Health.
 Home assessment – type of House, number of rooms, source of water, source of
Power, refuse disposal, excreta disposal, general cleanliness, basic diet, source of
food, domestic animals, resources available.
 Generate an action plan with justification based on the history, physical, home
assessment and literature review findings. The action plan should include
