◦ The aim of exchange transfusion is to lower the Hb S level to
40-60% while keeping the hemoglobin close to 10 g/dl.
◦ Ensuring that the following basic investigations had been
performed prior to admission (any investigations that had not been
done will be ordered by the primary team when patient is admitted
to the hospital).
◦ CBC,RFT LFT PT PTT, electrolytes
◦ electrophoresis
◦ Blood group and antibodies screen
◦ CXR, ECG, echo (if history of significant shortness of breath
and normal chest X-ray to rule out pulmonary hypertension).
◦ chest physiotherapy.
◦ (VTE) prophylaxis
◦ input-output charts and documenting results every 24 hours.
◦ Hematology consultation for high risk patients
◦ Antibiotics are to be given when clinically indicated, either
prophylactically or therapeutically.
◦ Patients undergoing splenectomy, preoperative immunization
with pneumococcal Vaccine, H. influenza, and meningococcal
vaccine should be administered 10 to 14 days preoperatively.
◦ Hydration with intravenous fluids: On the evening before
