• Assessment: observing, measuring (objective) and communicating
(non-verbal, appearance, cognitive abilities, other forms of
communication needed- subjective data is what the patient says)\

= Create baseline of patient through assessment such as Obs e.g BP,
Temperature etc but also what patient is telling you. Also waterlow assessment
and Must score, mental capacity assessment etc etc.

• Diagnosis: identifying the patients problems/concerns ( taking a
history-time frames, changes)

• Planning: planning care and setting patient-centred goals (realistic)/
aims *goal setting to be explored further later in the module*

= Desirable and achievable for patient

• Implement: being able to apply the nursing actions identified in
planning- interventions completing care plans -specific instructions –
e.g., dressing to be renewed every other day

• Evaluation: determining if the goals set have been achieved and if
needed reassessing.
