CBI continuous bladder irrigation Rx - bladder cancer - benign prostatic hypertrophic or prostate cancer transurethral resection of the prostate (TURP) for BPH - preferred method; fast recovery - less complications - removes inner portion of prostrate, via urethra, with use of endoscope - best for removal of small amount of tissue - spinal anesthetic - CBI often follows surgery Prostate cancer - Requires radical surgery and radiation - removal of prostate and adjacent tissue if tumour invasive - CBI may be indicated post op Bladder tumours Common in 50-70 yr men; as well as women May require transurethral resection of bladder tumour; cystomy and resection of bladder - CBI may be indicated post op Continuous bladder irrigation reason Continuous Bladder Irrigation: •To prevent clot formation and prevent obstruction •Slows bleeding (d/t coolness and pressure from fluid influx) •Often up to 24hrs post-op; depending on color of returns CBI purpose 1) Flushes clots and debris from bladder and urethra, to prevent urethral obstruction 2) The pressure from the force of flow of the CBI slows the bleeding how fast should CBI run? when to slow it down? - Initially fast, then moderate/slow, as returns become lighter in colour - Speed it up if returns become red or clots appear - slow irrigation as returns become pink or speed up rate as needed when to call physician for CBI Call physician if fever; severe burning; dysuria; bright red urine; blood clots in urine B & O suppository Belladonna: helps decrease bladder spasms that may occur while irrigation of the CBI setup Opium: Helps relieve the pain. if someone is experiencing pain within the urinary tract perform a CBI assessment (check for blockage & need for flushing - NEED PRN order before flushing) THEN if pain continues with spasm, administer B&O to relieve muscle spasms from irrigation and reduce pain
