LIGGING VAN HET RUGGENMERG
• Ruggenmergsegmenten genoemd naar wervellichamen waarlangs spinale
zenuwwortel uittreedt
o Segment ligt hoger dan corresponderende wervel
• Ruggenmerg omvat aantal langebaansystemen:
o Achterstrengen
o Tractus spinothalamicus
o Tractus corticospinalis/piramidebaan
o Begin perifere motorisch neuron = voorhoorncel
• Ruggenmerg begin bij achterhoofdsgat à eindig thv wervel L1
o Hoogste sensibele verzorging: dermatoom C2, gelegen over achterhoofd
+ onderrand kaak
o Niveauverschil onderste ruggenmergsegmenten-wervels met zelfde
segmentale aanduiding = ascensus medullae
§ Bv. wervel Th5 correspondeert met ruggenmergsegment
Th6-Th7; wervel Th12-ruggenmergsegment L3-L4
• Thv L1: sacrale segmenten = conus medullaris
o Cauda equina/paardenstaart = verzameling wortels die beneden
conus in spinale kanaal loopt
UITVAL DOOR EEN DWARSLEASIE
Blijvende volledige dwarslaesie van ruggenmerg op bepaald niveau (bv. C7 door
wervelletsel C6) leidt uiteindelijk tot:
• Beschadiging achterstrengen à uitval gnostische sensibiliteit vanaf corresponderende dermatoom
naar beneden
o Bv. C7: middelste deel onderarm en hand en verder naar beneden
• Beschadiging tractus spinothalamicus à uitval van vitale sensibiliteit in hetzelfde gebied
• Uitval reflexboog over getroffen ruggenmergsegment
o Bv.C7: tricepspeesreflex
• Beschadiging piramidebaan à verhoging reflexen onder dat niveau + pathologische
voetzoolreflexen ten gevolge van onderbreking piramidebaan + uitval kracht op en onder niveau
van dwarslaesie
o Bv. C7: pt kan armen nog buigen, maar niet meer strekken, handmotoriek is verloren,
benen zijn paralytisch
• Beschadiging voorhoorn à segmentale spieratrofie op niveau laesie door uitval motorische
voorhoorncellen
o Bv. C7: musculus triceps
• Beschadiging zijhoorn (met sympathische banen en celkolom) à vooral zweetsecretie en
bloeddrukregulatie gestoord
, • Uitval sympathische banen + besturing parasympathische centra in cauda à stoornissen
blaasfunctie en seksuele functies
o Parasympathicus voor de rest ongestoord doordat n. vagus niet is aangedaan
Onvolledige dwarsleasies: uitval kan grillig en asymmetrisch zijn, bv. onvolledige dwarsleasie thv C7 met
compressie vanbuitenaf:
• Gnostische sensibele uitval linksonder Th2 + rechtsonder Th6
• Vitale sensibele uitval van rechterbeen tot L1
• Spastische verlamming van beide benen
• Verlies blaascontrole.
