These use IA (Intermittent auscultation with a doppler or fetoscope). -All mom's and babies in the hospital typically need fetal monitoring In clinical practice the patterns of the Fetal Heart Rate (FHR) are described as - -reassuring: progressing normally with no intervention needed - non-reassuring: abnormal and in need of intervention IA (Intermittent Auscultation) - Listening to fetal heart sounds at periodic intervals to assess FHR IA can be performed with: - -Leff scope -DeLee-Hillis fetoscope -Ultrasound device Electronic Fetal Monitoring: External monitoring - -non-invasive -Fetal Heart Rate (FHR): ultrasound transducer -Uterine Contractions (UC's): tocotransducer -Can be used for woman are not termed Where is the Ultrasound transducer placed? - at the shoulder where is the Tocotransducer placed? - -at the top of the uterus to feel for contractions External monitoring disadvantages - -requires updating and repostioning as mother or baby changes position -As labor continues and baby moves down, the EFM will have to moved down Ultrasound transducer - -High frequency sound waves reflect mechanical action of the fetal heart. -Non-invasive, does not require rupture of membranes or cervical dilation and can be used during the antepartum and intrapartum periods for fetal monitoring. -As infant moves or labor progresses monitor must be adjusted.
