to be met. Outcome can be measured Qualitatively: How well does the learner
appear to understand the content? Quantitatively: How much of the content does
the learner retain?

Case management: Development and coordination of care for a selected client and
family.
· Involves process called disease management (A system of healthcare
interventions and communications for people with conditions in which client self-
care efforts are significant)
· Emphasizes prevention at the secondary and tertiary level using evidence-based

teaching to advance self-care and adherence to wellness care.
