Physical Activity Readiness Questionnaire (PAR-Q)


Regular exercise has many health benefits and completing the PAR-Q is a good place to start if you want
to boost your physical activity.

Physical activity should not be an issue or a hazard for most people. PAR-Q is intended to identify the
small percentage of adults for whom physical activity may be problematic, as well as those who should
seek medical guidance on the type of exercise that is most suited for them.

In answering these questions, common sense is the best guidance. Please read them carefully and, if
applicable, check the correct answer to the question.



1. Do you get chest pain when you engage in physical activity?

2. Do you have any bone or joint problems such as arthritis, which has
been aggravated by exercise or made worse with exercise?
3. To your knowledge, do you have high blood pressure?

4. To your knowledge, do you have low blood pressure?

5. Has your doctor ever said that you have a heart condition and that you
should only do physical activity recommended by a doctor?
6. Do you commonly encounter fainting spells, extreme dizziness, or loss

rest or after a light workout?
8. Do you work out fewer than three times a week?

9. Do you consume more alcohol per week than the average person (21
units for men and 14 units for women)?
