In deze ketenzorg opdracht heb ik beschreven hoe de Brijder kliniek en de DD-kliniek samenwerken als het gaat om overplaatsing van een cliënt. Er staat een beschrijving van beide afdelingen in. De voor- en nadelen van de overplaatsing en er is gebruik gemaakt van kwaliteitsindicatoren om de kwaliteit te meten. Ten slotte heb ik een aanbeveling gegeven die bijdraagt aan de verbetering van overplaatsing.
