- Voluntary Agency: A home health agency that is motivated by a not-for-profit
philosophy. → ex.) nurse appointed to a community-based advisory board of an
agency that is interested in providing fiscally sound, high quality care for clients they
decide will be their service population. Any profit margin that is acquired is reinvsted in
the operations of the home healthcare service.
- Case Management: involves disease management (emphasizes prevention aat
secondary / tertiary level using evidenced-based practice guidelines - can be a
nurse, social worker, public health expert). Development / coordination of care for a
selected client and family. Managed by RN.
- Client with type 2 diabetes & heart disease: provide weekly coaching sessions
by telephone for blood glucose management, demonstrate use of glucose meter
to client, review heart healthy / diabetic friendly food options with the client and
family.
