NIH Stroke Scale - Simplified and Added Cranial Nerves. Horizontal and vertical eye movements. Move pen or finger side to side, & up and down. Also test pupillary reflex to light. 0 = normal 1 = partial gaze palsy: eyes only move in one direction 2 = forced deviation: eyes do not move & are stuck in one position ________ N : III, IV, VI Concerned with eye rotation, movement, and pupillary reaction : Eye movement, pupil reaction 3. : Visual fields in upper & lower quadrants. Cover each eye and check other eye for peripheral vision with visual threat or counting fingers. *if patient moves eyes to see fingers, this is scored as normal* 0 = No visual loss 1 = Partial hemianopia 2 = Complete hemianopia 3 = Bilateral hemianopia (blind including cortical blindness) ________ N : Optic II Sense of vision in all fields. : Blindness over half vision field 4. F : Ask (or use pantomime) the pt. to smile, raise eyebrows, and close eyes. Score symmetry. 0 = N symmetrical movements. 1 = M (flattened nasolabial fold, asymmetry on smile) 2 = (total or near-total paralysis of lower face) 3 = C (absent face movements in entire face) ________ N : VII Facial, V trigeminal Concerned with facial movements and facial expressions : Facial paralysis
