Case Study 2: Myra McDermott and Elizabeth McDermott
INTRODUCTION:
In Section 1, I will be discussing how health and social care services meet the needs of each
individual chosen in my case study. Also, I will be discussing the barriers which might
prevent Elizabeth and Myra from accessing their chosen health and social care services.
For my assignment, I have chosen a case study that involves Myra McDermott, who is a
forty-two year old woman who lives with her daughter Elizabeth in a rural village. However,
the village has very few services. People have to travel into the nearest town for healthcare
and shopping. Myra has a skin condition which means that her skin is itchy and flakes
regularly. She needs to use creams provided by the G.P, and cannot sleep when her skin
becomes really uncomfortable. Myra also has gum disease and does experience bleeding
when she brushes her teeth. Myra is afraid that she will need to have all of her teeth
removed if she goes to the dentist and so has not made an appointment. Elizabeth
McDermott is twelve years old and attends the academy which is within walking distance
from home. Elizabeth is autistic and has behavioural difficulties. The head teacher at the
academy has told Myra that Elizabeth is becoming too difficult for the staff to manage. She
has said that the school will be discussing what to do with the Local Authority. This could
mean that Elizabeth is transferred to a school for children with additional needs. This means
that Elizabeth would need to travel by school bus to her new school. She will have to leave
her friends and travel, with other children she doesn’t know, to the new school which is five
miles away. Elizabeth’s family needs advice.
SECTION 1
There are many health and social care services ready to provide Myra with her specific
needs. The primary care dental team is diverse, including: dental therapists; dental
hygienists; and dental nurses. Specifically, periodontists are experts in the diagnosis and
treatment of gum disease which may result in periodontal surgery. Gingivitis is a very
common condition where the gums become swollen or infected. However, an appointment
has to be made with the dentist who will assess Myra’s dental needs. She will be asked to
provide information about her general health, including any medications she takes which is
confidential information. A surgery may be necessary if Myra has unhealthy tissue around
her teeth and cannot be repaired with non-surgical treatment. Fortunately, Myra will book
an appointment and may not result in her teeth being removed. Another solution Myra
could be comfortable with is having regular dental check ups so any problems with her teeth
are monitored and treated early. In most cases, her dentist or dental hygienist will be able
to give her teeth a thorough clean and remove any hardened plaque (tartar). Respect,
safeguarding and duty of care is used to meet Myra’s needs because the dental team is
supporting her without judging her, protecting her from hazards within the area and they
are doing their job to fulfill her needs. However, accessibility to the dental centre is difficult
(geographical barrier) so Myra can go to the nearest train station or bus stop to visit the

, dental clinic. If she is unable to do so, she can book a taxi through the internet and pay
which is the better option.
In addition, Myra could be experiencing Eczema (Atopic Dermatitis) which is a common
condition causing dry skin, flakes or severe irritation. The condition is also triggered by
eating dairy foods and nuts. A dietician (allied health professional) can offer advice on what
foods to eat for example, foods containing quercetin, probiotics and fatty fish. Gradually
reducing the eczema from developing. Atopic Dermatitis can be cured by an extensive range
of treatments. Pediatricians and primary care providers may provide an initial diagnosis,
prescribe treatments, or refer Myra to a pediatric dermatologist, dermatologist, or allergist
to ease Myra’s triggering symptoms. The allied healthcare professionals are showing the
care values of respect, confidentiality and person centred approach. A doctor will make a
direct diagnosis by examining the skin and reviewing Myra’s medical history. Another
solution, Myra could be comfortable with is applying creams that control itching and help
rebuild healthy skin cells provided by her G.P. People who are suffering with atopic
dermatitis are easily irritated by environmental triggers like soaps, laundry detergents, hot
or cold temperatures and stress according to the NEA. In some cases, a GP may refer Myra
to a specialist in treating skin conditions if Myra’s skin condition is unknown. Myra’s creams
provided by her local GP can be delivered to her house and she can use e-commerce to buy
silk pillows, duvets and sheets which can then be delivered to her home. A study published
in the Journal of Investigative Dermatology that surveyed nearly 35,000 adults revealed that
people with eczema had a higher chance of fatigue, daytime sleepiness, and insomnia. The
solution to Myra’s discomforting skin at night can be solved by simply changing the material
of the linens. Instead she can contribute to using clean and soft linens made of breathable
fabrics because they are suitable for people with Eczema.
Elizabeth McDermott is going through the early stages of Adolescence which has a severe
impact on her actions for example, throwing tantrums or having meltdowns. Counsellors or
mentors are the main solution to deal with behavioural issues because they are giving
advice about staying calm during difficult situations. Youth work may also be suitable for
Myra because she will meet many people, learn many things and expand her knowledge and
skills. Also, informal social care providers e.g. neighbours, relatives and friends can check up
on Elizabeth and Myra’s well-being. School staff are showing the care values of
safeguarding, duty of care, dignity and respect towards Elizabeth’s illnesses. Teachers can
offer help by talking about the bright side or offering rewards which will gradually improve
Elizabeth’s emotional activity. Common autism treatments include behaviour therapy,
speech- language therapy, play based therapy, occupational therapy and nutritional therapy
- provided by allied healthcare professionals. These treatments are maintained during a
routine, which will moderately improve Elizabeth’s behaviour. Elizabeth will be placed in a
secure place because she is going to be transferred to a high school for children with
additional needs. She will visit new people, make new memories and be placed in a new
environment with new things which will place Elizabeth out of her comfort zone. As a result,
she will get used to it and eventually enjoy her new surroundings. The school bus will take
Elizabeth to school and it will physically, socially and emotionally help Myra. She will feel
enlightenment that her daughter has the privilege to safely go to school.
