Unit 5 Assignment 2
Meeting Individual Care and Support Needs

By Kelsey McLeod

,P6: Explain why meeting the needs of the individuals requires the involvement
of different agencies
P7: Explain the roles and responsibilities of different members of the
multidisciplinary team in meeting the needs of specific individuals
M5: Assess the benefits of multidisciplinary and multi-agency working for
specific individuals with care and support needs

In this section, I'll look at various experts, as well as the agencies and organisations
around them, may meet people's demands and how information needs to be shared
and handled properly. I'll examine the relevant laws and codes of conduct before
discussing how everything mentioned above serves to support people, keep them
secure, guarantee the greatest results, and assist with any potential conflicts of
interest.

How agencies work together to meet individual care and support needs
According to research, people find it difficult to maintain their health in places
with significant unemployment, housing problems, and a dearth of public
transportation. The lack of coordination across agencies has historically been one
of the biggest issues with health and social care. A family in a deprived area of the
UK can correspond with tens of organisations simultaneously and tell the same tale
to each organisation. A year later, individuals might discover that they are still
telling their experiences to the same agencies. They could receive services that are
imposed upon them as opposed to organisations collaborating with them to suit
their needs, with little to no improvement in their health or wellbeing. In order to
increase the system's effectiveness and efficiency, there has been a push to
combine services in recent years.

A multidisciplinary team, as described by Cambridge Dictionary, is one that
includes or relates to individuals from several fields of expertise. A
multidisciplinary team is made up of professionals working for the same
organisation in the fields of health and social care, where each plays a specific role
in helping a service user. These experts collaborate to provide assistance to a
person or family dealing with challenging circumstances. If a service user is
getting better treatment and results from their service providers, it follows that their
multidisciplinary team is functioning successfully and efficiently. As each team
member must work together to be aware of potential conflicts between the service
provider and user and to ensure that the service user's requests are honoured, a
multidisciplinary team also helps prevent duplication of roles and duties.
The qualities of a multidisciplinary team can be broken down into five key
components. A multidisciplinary team should initially be composed of individuals
who each have unique talents and expertise that may be pooled for the service
user's wellbeing. The standard of care that a service user is receiving may be
harmed if a multidisciplinary team is formed but only includes members from one
speciality. This brings up the second argument, which is that the goal of a
multidisciplinary team is to operate under common principles. This means that
each team member must adhere to a set of fundamental principles. For instance,
each member of a multidisciplinary team should desire the care plan for the service
user to be successful. The third aspect of a multidisciplinary team is that each
member should be able to pick up new abilities from their colleagues who have
unique talents of their own. Within health and social care, the capacity of service
providers to enhance their own methods of providing care is essential. Without it,
service customers receive care that is of lower quality, which results in support
failures. Collaboration and co-creation are the members of each multidisciplinary
