,VOORWOORD

Voor u ligt het gezondheidsbevorderingsplan gericht op een zorgvrager met COPD. In dit plan wordt
een casus uitgelicht waarbij het gedrag van de zorgvrager niet correspondeert met belangrijke
zelfmanagementtaken die horen bij het gezondheidsprobleem. De persoon in kwestie beïnvloedt
daarmee het verloop van haar gezondheidsprobleem. Er wordt in het plan aandacht besteed aan met
name zorggerelateerde preventie en preventie op tertiair niveau. Het gezondheidsbevorderingsplan
is geschreven tijdens de PLP-bachelor van verpleegkunde te Windesheim. De PLP-bachelor is
uitgevoerd in de wijkverpleging bij de organisaties *** en ***

Dank gaat uit naar de werkbegeleiders ***, *** en ***. *** willen wij bedanken voor de
informatieleerwerkgroep lessen met duidelijke feedback en prettige begeleiding. Verder willen wij de
longverpleegkundigen, evenals de reguliere verpleegkundigen binnen de organisaties bedanken voor
de inbreng van de expertise. Tot slot willen wij de zorgvrager in kwestie bedanken voor de
medewerking aan dit plan.

Het schrijven van dit gezondheidsbevorderingsplan was een uitdagend en dynamisch proces, wij
kijken dan ook met trots terug naar de afgelopen periode en dit eindresultaat.
