Pedigree Assignment

John Doe is a 43 year old heterosexual Caucasian male who is married with three older

children. He is a close personal friend and was kind enough to allow this inquiry. The interview

was conducted on January 15, 2020 at 1023, obtained from the patient as he is considered a

reliable historian.



CHIEF COMPLAINT

Mr. Doe’s is mostly concerned with his weight. He is obese with a body mass index

(BMI) of 37.30. Mr. Doe claims he has been struggling with obesity his entire life, starting in

early adolescence. He states that the only time he has not been obese was during his eight year

tour in the U.S. Army. He believes this is due to their strict weight requirements and mandatory

exercise programs. Mr. Doe admits that he has not been eating healthily and has been living a

sedentary lifestyle for at least the last five years or more. He states that this is the largest he has

ever been and feels most comfortable between 200 and 240 pounds.



PATIENT HISTORY

Mr. Doe suffered from severe eczema on both hands and arms in late adolescence to early

adulthood. The rash would start between his fingers, then spread up his arms all the way to the

axilla if left untreated. It was exacerbated by excess body sweat. The eczema was treated

topically with Triamcinolone 0.1% cream with great success. Mr. Doe reports that he has not

had a breakout since his mid-twenties.

Mr. Doe had his gallbladder removed in 2015 after reporting a sharp right-upper-quadrant

pain just under the ribs. The HIDA-scan showed a “sluggish” gallbladder. Mr. Doe reports that
