Direct and consensual pupillary response is equal Visual fields Visual fields are full in all four quadrants Extra Ocular Muscles Corneal reflection is equal bilaterally, which shows no sign of strabismus. Extra ocular muscles are parallel and conjugate, which indicates an absence of nystagmus Fundoscopic Exam Presence of red reflex, optic disc is clearly visible and is yellow-orange in color, appropriate cup to disc ratio of 1 to 2, no evidence of any AV nicking, copper wire arterioles, silver wire arterioles, or hard or soft exudates. And the macula is visible when pt looks directly into fundoscopic light. Inspect Ear Auricle and surrounding tissues do not exhibit any deformities or lesions. Auricle and tissues are a norm pigmented pink color. With palpation of the auricle, tragus, and mastoid process there are no signs of tenderness Auditory Acuity Pt was able to verbalize both sequences correctly, which indicates that hearing is intact bilaterally Webber Test Pt hears tuning fork equally in both ears, indicating that hearing is intact bilaterally. If sound was heard only unilaterally in the ear that is occluded, that would indicate a conductive hearing loss. If the sound was heard unilaterally in the clear/good ear, then that would indicate a sensorineural loss. Rinne Test Bilaterally, Pt hears tuning fork in ear canal longer than at mastoid process. This indicates that hearing is in tact bilaterally. If pt were to hear the sound longer at the mastoid process, this would indicate a conductive hearing loss
