,1. INLEIDING SCHOUDER
SITUERING




SCHOUDER: FYSIOPATHOLOGIE

• Artritis Glenohumeraal (GH) of Acromioclaviculair (AC)-G:
o Post-immobilisatie/ -artoscopie
o Posttraumatisch/ degeneratief OA
o Frozen shoulder

• Subacromial pijnsyndroom (SAPS):
o = Subacromiale “inklemming” van weke delen: bursa, supraspinatuspees, bicepspees, …
o Geeft dikwijls GH-bewegingsbeperkig met soms scapulothoracale en/of CTO
functiestoornissen
• Schouderpeesproblematiek:
o Dikwijls tendinopathie soms met calcificatie en scheurvorming
o Soms traumatisch
o Dikwijls t.g.v; SAPS
• (Anterieure) instabiliteit GH-G:
o Meestal post-operatief of post-immobilisatie

SCHOUDER: FUNCTIESTOORNISSEN

• Verminderde mobiliteit
• Verminderde stabiliteit (ligamentair-musculair)
• Verminderde comrpessie-BBH
• Pijn & stijfheid (klinisch) tijdens houding en/of beweging
• Referred pain:
o GH: uitstralende pijn in C5-T1 (meestal C5-C6)
o ACG: vnl. pijn in C4-C6 gebied
• Krachtsverlies

,SCHOUDER: KINEMATICA

• Caput humeri staat iets naar posterieur en craniaal gepositioneerd
• Belangrijk:
o Controle van caput humeri naar caudaal en posterieur tijdens bewegen (vnl. bovenhands)
o Belangrijke interactie van GH-G met scapulothoracaal G (Upward rotation, posterior tils,
exorotatie of protractie)
• Rotator cuff:
o Verzorgt posterieure translatie van caput humeri tijdens opwaartse bewegingen door
supraspinatus en infraspinatus
o Verzorgt inferieure translatie van caput humeri door infraspinatus en subscapularis

SCHOUDER KINEMATICA: ANTEFLEXIE-ELEVATIE

• GH-G:
o Tot 90° abductie: anterieure translatie humerus
o Vanaf 90° abductie: posterieure & inferieure translatie humerus
• Scapulothoracaal-G:
o Upward rotation: 0°-180°
o Posterior tilt: vanaf 120°
o Exorotation: vanaf 120°




• Exorotatie in 90° abductie: posterieure translatie humerus
• Max. exorotatie in 90° abductie: anterieure translatie humerus
• Max. endorotatie in 90° abductie: posterieure translatie humerus

,KLINIMETRIE SCAPULABEWEGINGEN & SAPS: EBP

Zijn scapulabewegingen gestoord bij P met SAPS?
è CAVE: Studies kinematica & EMG vs. studies klinische veldtests!
Systematic review Struyf et al.(2014): EMG SAPS VS controle:
- Verminderde EMG-activiteit in Lower Trapezius en Serratus Anterior
Systematic review Kinsella et al.(2017) : EMG SAPS VS controle:
- Tendens tot verminderde EMG-activiteit Serratus Anterior (< amplitudo; te late reactie)
- Ratio Upper Trapezius tov Mid & Lower Trapezius-activiteit te groot
- Verminderde activatie van infraspinatus, supraspinatus, subscapularis tijdens 1ste fase van scapulaire
abductie.
Systematic review Ratcliffe et al.(2014) :
- Geen evidentie beschikbaar dat kinematica effectief gestoord is bij SAPS
- Geen valide klinische test beschikbaar om scapulaire dyskinesis vast te stellen
Systematic reviews Lange et al.(2017) en Dhondt et al.(2021) :

- Klinische tests voor scapulaire dyskinesis zijn onvoldoende betrouwbaar
- “There is currently insufficient evidence to recommend any instrument for the clinical examination of
scapular function. Measurement instruments to assess scapular dyskinesis are prone to
misinterpretation and should therefore not be used as such”.

JOINT-PLAY GH: ABDUCTIE-ELEVATIE

è TRANSLATIE NR CAUDAAL
Doel:
P SFO: Joint-play onderzoek GH-G nr abductie-elevatie
P Mate van translatoire mobiliteit nr caudaal nagaan
P Rekbaarheid inferieure G-kapsel nagaan
9 Bij abductie-elevatie is het belangrijk dat er een lichte translatie gebeurt nr caudaal, dit kan
alleen als er voldoende elasticiteit is in het inferieure G-kapsel.
P Indicatie: Bij een duidelijke bewegingsbeperking nr abductie-elevatie (BFO).
Uitgangshouding:
- P in zit op krukje, mooi recht, voeten gesteund
- Arm gesteund op kussen, t.h.v; tuberositas deltoidei
- Breng de arm in de actuele eindstand
9 Feedback: scapula begint te veel mee te bewegen + afweersapnning/ rekspanning
- Arm in neutrale positie
9 In EXO meer rek op anterieure G-kapsel
9 In ENDO meer rek op posterieure G-kapsel
Handvatting:
- NGH: Op margo superior scapulae à stabilisatie, nt fixatie
- GH: Palmaire deel vh zwemvlies lateraal vh acromiale uiteinde op het caput humeri

Uitvoering + beoordeling:

FASE 1: TRANSLATIE

- GH: Slack opnemen + translatie toevoegen (duwen en lossen, slack nt losssen)
o Translatierichting: caudaal – iets lateraal
- Beoordeling: Mate van translatie nr caudaal nagaan à L-R vgl

FASE 2: TRANSLATIE KOPPELEN AAN ANGULATIE

- Handvatting aanpassen:
- GH: Omvat voorarm en distale deel vd bovenarm
- NGH: Palmaire deel vh zwemvlies lateraal vh acromiale uiteinde op het
caput humeri
- Slack opnemen + translatie toevoegen
- Angulatie nr abductie-elevatie toevoegen
- Beoordeling: Nagaan in welke mate de abductie-elevatie bevorderd wordt door
de translatie nr caudaal
- Beoordeling: Graad van afweerspanning nagaan à brug nr therapie
