Subjective Data Chief Complain (CC): Left hip pain x 3 months History of Present Illness (HPI): L.S. is a 60-year-old male here today with a complaint of left hip pain that began suddenly and has lasted x 3 months, described as constant “aching and burning” in the lateral and anterior regions and radiating down his left buttock and into his calf, especially when standing or walking for long periods. He rates the pain 8/0-10 scale and denies any recent fall or known trauma. Pain is somewhat improved after prolonged rest, use of heating pad, and Ibuprofen for pain. Associated symptoms include tingling in lower left extremity and difficulty sleeping. Significant hx of hip injury approx. 30 years ago when he fell from a ladder onto his left side. He did not seek medical attention and reports a full recovery with no long-term effects. Current everyday smoker, but no other significant medical, surgical, or family hx. Medication use is limited to Ibuprofen PRN. Allergies: Penicillin-rash Past Medical History: Previous left hip injury 30 years ago, resolved without medical intervention. Otherwise, unremarkable.
