, 1 Oriëntatie therapie
1.1 Zorgcontinuüm
Fase 0: brede basiszorg
- Leerkracht stimuleert de totale ontwikkeling van alle kinderen via
een “krachtige leeromgeving”
o “Wat alle leerlingen nodig hebben om zich te kunnen
ontplooien en gebruik te kunnen maken van hun talenten
en mogelijkheden”
o Voor alle lln dezelfde brede basiszorg, met aandacht voor
de noden van elke leerling
- Leerkracht tracht problemen te voorkomen
o Preventieve zorg
 Proactieve en preventieve acties

Fase 1: verhoogde zorg
- Het zorgteam van de school bekijkt samen met leerling, ouders en leerkracht hoe de leerling
op school extra zorg kan krijgen




- Welke zorg?
o Remediërende, differentiërende, compenserende of dispenserende maatregelen
- Door wie?
o Bij voorkeur uitgevoerd door de klasleerkracht
o In samenspraak met het zorgteam
o In open communicatie met ouders en met leerling

Fase 2: uitbreiding van zorg
- Beter zicht krijgen op het functioneren van een kind binnen zijn context
- Om zo een beter aanbod te kunnen bieden, afgestemd op de zorgvraag van de lln
- Samen met leerling, ouders, schoolteam, CLB en eventueel andere actoren
- Individuele leerlingbegeleiding door het CLB: verschillende vormen:
o Een handelingsgericht diagnostisch traject
o Informatie- en adviesverstrekking zonder HGD-traject (bv.
onderwijsloopbaanbegeleiding)
o Kortdurende begeleiding
o Samenwerking met een netwerk

Fase 3: IAC: individueel aangepast curriculum
- Leerling kan het gemeenschappelijk curriculum (GC) onvoldoende volgen: specifieke
onderwijsbehoeften  aanpassingen in de school en/of klas
- “Verslag” opgemaakt door CLB, lln, ouders, school met keuze:
o IAC in huidige school voor gewoon onderwijs, mits akkoord school
