Lecture Case 6 Innovations in chronic care
management IQM
Background chronic disease

Why is chronic disease important?
-Incidence of chronic disease is increasing (causing healthcare expenditures)
-Frequency and scope (burden of patients) is increasing
-Different need/approaches of care for chronic disease (because it is life time long)
-Innovation might be able to tackle or reduce this

Non-communicable diseases kill slowly in normal times and quickly in covid-19 diseases.
-People who have chronical disease tend to have a worse outcome when having COVID-19
-Chronic diseases therefore offer challenges to health systems

WHO Definition of chronic disease: two important elements
-Long term health problems that are not life-threatening and you can life with
-Tend to slowly progress over time (get worse and will impact physical and mentally functioning)

Multiple chronic diseases are known such as: diabetes, depression, arthritis.

How do chronic diseases differ and can you see communalities between conditions based on their
nature (e.g. onset, course and impact)?

4 categories of chronic diseases:
 Life threatening conditions:
 Intermittent conditions: depends on period how severe it is (with stable periods and flare
ups with exacerbation in disease burden e.g. asthma)
 Progressively debilitating (patient worsening over time e.g. dementia)
 Chronic mental illness (e.g. depressions and dementia)
 Can be overlap between these categories like in case of dementia
 Different impacts on patients, caregivers and family

What drives prevalence of chronic diseases?:
-As population has aged, we see more chronic disease (tend to develop at later age)

Risk factors of chronic disease:
 Diet
 Physical inactivity
 Obesity
 Alcohol and tobacco use
 High cholesterol and high blood pressure
 Genetics
 Social environment

Air pollution is now also important in prevalence of chronic conditions

Diseases that used to be deathly (hart failure, diabetes and HIV), can now be treated with medicines
and technological innovations.
