The perceived health need I identified is Alcohol/Substance Abuse. The population of
target within the community are those facing economic insecurity or low income. I would
target this population because it crosses ethnic and cultural lines thus touching a larger
population within the community. There is a direct correlation between poverty and
Substance abuse such as increased stress, feelings of hopelessness, lower self-esteem, and
reduced social support (Nccaa, 2020).
In reviewing healthy people 2020 I identified the following potential funding resources:
State and local health departments, Charities/Foundations, and Local businesses
(Healthypeople.gov 2018). However, it also made me think about other available resources
such as Substance Abuse and Mental Health Services Administration (SAMHSA) which
offers the Catalog of Federal Domestic Assistance (CFDA) which is a listing of programs
available to not only state governments but also to local, tribal, and territorial governments.
In addition, Grants.gov and HHS grants information on policy and administration are
provided along with tips for applying, administration tools, and the evaluation processes
(Samhsa Resource Database,2021).
In choosing partners to mobilize with regarding a program to address Alcohol/Substance
abuse I would look for those with ties to the community and county for participation. My
reasoning would be based on how they provide service to the community, what insights they
have gained from previous community participation, and they may have established
programs that will provide insight on what works, what can be improved upon, and a pool for
community volunteers and/or potential staffing. Of the potential partners as provided by
Health People 2020, local business I would choose Target and MedStar Health System as
