Millie Larsen, Part 2 Guided Reflection Questions Opening Questions How did the simulated experience of Millie Larsen’s case make you feel? This experience was difficult for me. Millie was clearly coming out of her confused state. She was alert and oriented. However, her answers differed from her daughter. That made the assessment tricky for me. I had to base it off what the daughter said instead of Millie. What nursing actions did you feel were appropriate within this scenario? It was appropriate to complete vitals and a general head to toe assessment. It was necessary to assess her functional status as well as her confusion. Scenario Analysis Questions* EBP What were at least three nursing actions you performed for Millie Larsen, in order of priority, and why did you perform them? I assessed the vital signs. Even though this patient was not in immediate danger, it is always important to check vital signs. There could be an unnoticed issue. I assessed the patient’s confusion level. This was important to check her progress. She came in very confused and it was necessary to see how that had changed. I performed a functional assessment of the patient. This was important to tell us how much help Millie would need after discharge. It allowed the healthcare team to know that she would not be able to discharge home immediately without assistance. EBP What potential problems can you identify for Millie Larsen? She is at a significant risk of falling if left alone. She is also at risk for missing a medication dose by herself. She will need home help to be safe. She is at risk for getting another UTI. PCC What communication techniques should be used to improve the conflict between Millie Larsen and her daughter? The nurse needs to respect both Millie’s desire to be independent and Dina’s desire to protect her mother. The nurse should help both to realize that they are looking out for each other and both have valid concerns. Being open and honest is important. Addressing the issues head on in a calm and reasoned manner will help.
