NUR 3225 Mock NCLEX exam Frequently Asked Questions And Final Exam Predictor Mock NCLEX exam Frequently Asked Questions I have a question about #74. I understand why the first choice is wrong, the second choice has nothing to do with BP, but the third (which was given as the correct answer) says to change the position to side-lying. Isn't one of the first things we are supposed to do is to have the patient lay on their left side if they are hypertensive in order to decrease the BP? If their BP is already low, then I don't want it to drop more...HELP!!! If you put the patient on her left side then this will increase the blood flow to the placenta and thus the baby... since the BP dropped... the CO output dropped and this would mean that the baby would not be getting the blood that the baby needs. I understand that you have to get more right than wrong...but there has to been a % more right than wrong.. If I take 75 ?'s and I get 45 right does that mean that the computer should shut off? There has to be a point when the computer says that it is more right than wrong??? My first time doing the MOCK NCLEX I got 91/125 that is obviously more right than wrong but is not that great. So how would you say that 91/125 is? THANK YOU! I know you want to have a % BUT the NCLEX is not graded with a %.... you have to get more right than wrong...Again...Well... if you take 75 questions... and remember you will only be "graded" on 60 of those b/c 15 (if you are taking the RN test) are pre-test questions that dont count for or against you and you dont know which questions they are.... you would have to get more right out of those 60 than wrong... so yes... getting 45 out of the 60 that you are graded on should pass you on the test...but remember you have to take the min of 75 questions....so dont think just b/c you took 60 questions you are done... the computer will shut off when you have completed the test or time has run out on you. how am I suppost to study or choose the right answer for drug questions like adverse reactions? Remember... you are not trying to get them all right. This is also a low level question, which means there is nothing in the stem of the question to help you. Unfortunately, you just have to make the best educated guess you can. Question 42 asks us to facilitate interaction between these two clients. Wouldn't going to a unit activity (#3) just distract them from this outcome? Putting a puzzle together (#4) with the clients allows the nurse to facilitate conversation and interaction between the 2 in a non-threatening manner. Thx. 42. The nurse is assigned to a room with two clients. Both clients are diagnosed with depression and spend most of their time in their room, but do not talk to each other while there. The nursing action that would be most effective in facilitating interaction between these two clients is to: 1. Suggest that they watch television together in their room. 2. Explore with each other their reluctance to engage in conversation. 3. Take them to a unit activity together. 4. Put a puzzle together with them in their room. Two depressed clients…..trying to get them to interact with This study source was downloaded by from CourseH on :09:01 GMT -06:00 This study resource was shared via CourseH each other #l….you can watch television and never say a word #2…..”Explore” sounds so therapeutic…….Why don’t ya’ll want to talk to each other?....I don’t know, I’m depressed #4….Again, we are doing something that doesn’t require conversation #3…Get them to doing something active. Activity makes people talk! hei, in reference to question 81, about the CABG post-op who is emotionally drained, your rationale was based on suicidal thinking that's why answer 4 was right. However, if you analyze the question critically, it does not show any indication of suicidal feelings from the patient, so why that answer Your hints in the question: Recovery has been very difficult and emotionally draining. and....the client reveals it may be better for everyone if she doesn’t go home at this time. So it then asks for the priority.. killer answer....assume the worst question 99: Okay, I get that we don't eat crickets in the US but wouldn't depending on prayer ALONE to handle a schizophrenic "problem" be more of a concern in non compliance to his medication regimen than eating crickets that won't really hurt him? 99. The nurse is discussing discharge plans with a patient diagnosed with schizophrenia. Which statement by the patient requires immediate attention by the nurse? 1. “Crickets will be great for increasing my fiber.” 2. “I still hear voices on a regular basis.” 3. “Prayer alone can cure my problem.” 4. “I’m worried about being cold this winter.” Answer: #l…This patient is telling you they are going to eat crickets….You need to stop right now and say What? I KNOW what some of you are thinking……..Some people eat crickets. NOT as a rule…This is not a common occurrence #2. They are schizophrenic they are going to hear voices. They won’t ever be cured. We can only hope to decrease the symptoms #3. There is nothing wrong with prayer…..some of you were scared they weren’t going to take their medicine, so talk to them about it but there is something more immediate and BOLD #4. Not life threatening…there’s something more bold I am not sure what kind of score or rating I need to know I will pass Nclex. Is there a particular score or rate? Thank you No, you are just trying to get more right than wrong. The NCLEX is not graded as the test were in nursing school. There is no percentage you need to get right or wrong to pass. for question 114,wouldn't a decreased BP indicate bleeding? You dont know their baseline BP... also since bleeding is the #1 complication post heart cath... and I see blood... I assume the worst... this patient is obviously bleeding... and this could kill them Where are the questions for the mock exam and can they be printed out. The are located under 5th day bonus material listed as HURST REVIEW Mock NCLEX Exam This study source was downloaded by from CourseH on :09:01 GMT -06:00 This study resource was shared via CourseH For Question #99. I chose the answer to be #4 thinking that they may have a problem with lodging for the winter. I was thinking homeless. May need to be referred to a social worker. Am I reading too much into the question? I still don't get the Crickets part, can you explain a bit more. Which statement requires IMMEDIATE attention! #2. Not life threatening…there’s something more bold #l…This patient is telling you they are going to eat crickets….You need to stop right now and say What? I KNOW what some of you are thinking……..Some people eat crickets. NOT as a rule…This is not a common occurrence IAM TAKING THE ON LINE VERSION OF THE REVIEW. HOW CAN I TAKE THE MOCK EXAM? The are located under 5th day bonus material listed as HURST REVIEW Mock NCLEX Exam Where can I access the mock NCLEX exam to print out; I cannot find it! The are located under 5th day bonus material listed as HURST REVIEW Mock NCLEX Exam so from what I am hearing you say--if I got 95 out of the 125 questions on your mock exam then I should feel pretty good? I am just really nervous. yep. what is an arteriogram An arteriogram is also called an angiogram. An arteriogram is an x-ray image of the blood vessels to evaluate various vascular conditions, such as an aneurysm (ballooning of a blood vessel), stenosis (narrowing of a blood vessel), or blockages. where do you get the questions at for this review? The questions are located in the 5th day bonus material listed as "Mock NCLEX". question # 9... I want to pick both output & VS. Why is output more important than baseline VS signs K can cause heart issues and the pt is dehydrated which can cause both Kidney and Heart issues...Thank you...love this course!!! First Underline Dehydration and KCL They think since the patient is dehydrated you are going pick #1, #3, or #4 These are the first things we think of with dehydrated patients Look for something life threatening……always on NCLEX You’ve got to tell the NCLEX people that you know that urine output is VERY IMPORTANT when you are giving KCL. Because we excrete potassium through our kidneys #2 is the BEST answer because if the patient does not have good This study source was downloaded by from CourseH on :09:01 GMT -06:00 This study resource was shared via CourseH output and you GIVE KCL….you could kill the patient with a life-threatening arrhythmia. b/c the patient will be retaining potassium Nothing is wrong with weight, or skin turgor b/c they are dehydrated Nothing is wrong with getting the VS All 4 answers are fine, but the one that could kill your patient is the answer…. If you did not assess the UOP.. if the UOP decreased.. Your patient would be retaining potassium and the patient could have a life threatening arrhythmia. Question 67 says to give the patient 1/2 normal saline (a hypotonic solution) for hypertonic dehydration. However the notes I took for hypernatremia said to give the patient an isotonic solution so the fluid stays in the vascular space...I thought a hypotonic solution would leave the vascular space. Are my notes wrong? 67. The client has hypertonic dehydration. Which of the following solutions should the nurse anticipate will be ordered by the physician? 1. Lactated Ringers 2. 0.45% saline 3. 10% Dextrose in water 4. 5% Dextrose in normal saline Hypertonic dehydration What does hypertonic mean? Packed with Particles If they are dehydrated you know their blood is full of NA Hypernatremia=dehydration So we have a patient that needs fluids, but not the NA because they are already full of sodium #l. LR…….has a lot of NA #3. Okay, but not Dl0 #4..anything with Normal Saline in it has sodium #2…this solution has HALF the sodium 1/2 NS is a hypotonic solution... it will rehydrate... but will just not stay there... On question 43 regarding the VP shunt and discharge teaching I picked the abdomen for distention. If the shunt drains fluid and the shunt fails then the fluid can accumulate in the abdomen. Why would this not be taught to the parents as a precautionary measure to prevent further complications? 43. The nurse has completed discharge teaching for the parents of a one-month-old infant who has had a ventriculoperitoneal shunt inserted. The nurse evaluates that the teaching was effective when the parents state: 1. “A new shunt will probably be needed in a couple of years.” 2. “We will check the abdomen daily for signs of fluid accumulation.” 3. “The doctor will need to be notified if urine output decreases.” 4. “We will cleanse the catheter site daily using sterile technique.” This patient has had a shunt inserted to drain off excess spinal fluid from the brain…. #2…..check the abdomen….this is not enough fluid to effect the girth #3….not enough fluid to affect the urine output #4…..there is no catheter site #l….they will have to put a new shunt later because as the child gets taller a new shunt will have to be inserted online question #94 depressed psych pt grooming is improved. he is wearing clean cloths and combed hair. the correct answer is complimenting him #4. Why is the answer #3 wrong asking him what the special occasion is my thinking is suicide depressed pt A client is admitted to the psychiatric unit for depression. The nurse observes an improvement in the client’s grooming when the client comes to breakfast freshly bathed wearing clean clothes and with her hair combed. Which of the following responses by the nurse would be the most therapeutic? 1. “You must be getting better—you look great.” 2. “Let’s go put some make-up on to make you look even better.” 3. “Why did you get all dressed up today? Is it a special occasion?” 4. “You look very nice in your clean dress after your bath and shampoo.” A depressed patient who has done something positive #l. This is assuming they are getting better….They This study source was downloaded by from CourseH on :09:01 GMT -06:00 This study resource was shared via CourseH might be about to kill themselves #2. You look good, but you could use some lip gloss! #3..Never say WHY when you want to be therapeutic #4. ACKNOWLEDGES a positive action ? on #119. I read this as looking for a initial priority (short term goal). Choices 1, 2 & 4 all see like they would be long term goals. Am I reading into this too much? 119. The primary nurse learns that an obsessive-compulsive client has a full set of dentures because he eroded all his tooth enamel with his brushing rituals. He also brushes his tongue several times a day, and has developed several ulcerations on it. His nursing care plan should set the highest initial priority for which of the following? 1. Eliminating his brushing and mouth care rituals. 2. Verbalizing the underlying cause of his behavior. 3. Seeking out the nurse when he is feeling anxious. 4. Reestablishing healthy tissue in his mouth and tongue. You want to reestablish the healthy tissue b/c that could lead to an infection and be harmful to the patient. It isn't asking for a short term goal.. it is asking for what would you do first and your answer has to prevent harm... ummm where is the actual mock nclex?? all i can find is the video with the answers... The questions for the mock NCLEX are located in the 5th day bonus material and is called HURST REVIEW Mock NCLEX Exam SO in number 67 are you saying that D10 is the answer, because I thought you said that D5 in normal saline was the correct one because it had the least amt..... I'm confused or maybe I heard wrong.. 67. The client has hypertoni dehydration. Which of the following solutions should the nurse anticipate will be ordered by the physician? 1. Lactated Ringers 2. 0.45% saline 3. 10% Dextrose in water 4. 5% Dextrose in normal saline Hypertonic dehydration What does hypertonic mean? Packed with Particles If they are dehydrated you know their blood is full of NA...Hypernatremia=dehydration So we have a patient that needs fluids, but not the NA because they are already full of sodium #l. LR…….has a lot of NA #3. Okay, but not Dl0 #4..anything with Normal Saline in it has sodium #2…this solution has HALF the sodium im trying to locate how to get the questions for the mock nclex exam.. i have the dvd which show the answers and rationales and i also looked thru the complete set of notes content and cant seem to find those questions. thank you The Mock NCLEX is located under 5th day bonus material listed as HURST REVIEW Mock NCLEX Exam On question #4 what is a audiometry? I've never heard of that before. Audiometry is the testing of hearing ability. Typically, audiometric tests determine a subject's hearing levels with the help of an audiometer, but may also measure ability to discriminate between different sound intensities, recognize pitch, or distinguish speech from background noise. Acoustic reflex and otoacoustic emissions may also be measured. Results of audiometric This study source was downloaded by from CourseH on :09:01 GMT -06:00 This study resource was shared via CourseH tests are used to diagnose hearing loss or diseases of the ear, and often make use of an Audiogram. Is apple juice and ginger aid considered to be full liquids. i thought they were clear liquids. how can i differ clear from full liquids if its on the test. please help!! Apple juice and ginger ale are both clear liquids.. clear liquids are any liquid you can see through. I could not find the appropriate place to place this comment. However, I wanted everyone to know (especially Hurst Staff) that I have PASSED! my NCLEX exam on the first try. I am soooo thankful and greatful that the lord has led me to this review. I had a lot riding on me passing the NCLEX on the 1st time. I am soo proud and honored to say that I am apart of the ARMY NURSE CORPS. I was commissioned to a 2nd Lieutenant and I owe this accomplishment to the HURST REVIEW. THANK YOU SO MUCH!!! and GOD BLESS! Congrats! :) Okay, here we go again with question #99. I see other people have questioned this too. I picked #3 for an answer, "Prayer alone can cure my problem." Yes, I am worried that they are going to go off meds, isn't this the primary concern (w/ the exception of ineffective communication?) In my mind this is "the killer answer." And on this question it's the first time I have heard anything about "bold" being used as a test strategy? Could you please clarify this for me? Thank you. I LOVE the fireplace, btw! ;) When it says BOLD it is talking about.. WOW.. eating crickets is not a norm... it is a little unique..... 99.The nurse is discussing discharge plans with a patient diagnosed with schizophrenia. Which statement by the patient requires immediate attention by the nurse? 1. “Crickets will be great for increasing my fiber.” 2.“I still hear voices on a regular basis.” 3.“Prayer alone can cure my problem.” 4.“I’m worried about being cold this winter.” Answer: #l…This patient is telling you they are going to eat crickets….You need to stop right now and say What? I KNOW what some of you are thinking……..Some people eat crickets. NOT as a rule…This is not a common occurrence #2. They are schizophrenic they are going to hear voices. They won’t ever be cured. We can only hope to decrease the symptoms #3. There is nothing wrong with prayer…..some of you were scared they weren’t going to take their medicine, so talk to them about it but there is something more immediate and BOLD #4.
