P6 - Explain why meeting the needs of the individuals requires the involvement of
different agencies.

In this assignment, we will be looking at the different agencies and why their
involvement it’s important in order to meet Valerie B., Aisha H. and Billy G. needs.

Valerie
For Valerie, I choose to involve the role of bodies responsible for integrating health
and social care., which is an agency responsible to put in place arrangements that
integrate health and social care settings in order to come up with a good outcome for
carers, service users and their families. Furthermore this act requires the involvement
of the health boards and local authorities working together to effectively deliver a
good quality and integrational care service for the service user. As Valerie has
underlying health conditions that affects her health and wellbeing the role of bodies
responsible for integrating health and social care offers the Health and Wellbeing
Boards (HWB) which are statutory bodies introduced in England under the Health
and Social Care Act 2012, that promote and improve integration between practitioner
in local health care, social care, public health and other related public services., by
aiming to deliver a better “joined up” care for service users, this board also has a
statutory duty with the Clinical Commissioning Groups (CCGs) to produce joint
strategic needs assessments and a joint health and wellbeing strategy for their local
population. In Valerie’s case, the Health and Wellbeing Boards will be working in
partnership with the NHS, public health and local government by making sure that
her needs are being met within her local area, as Valerie has mental health issues and
suffers from obesity and financial problems. The Health and Wellbeing Boards will be
joint with the local government to ensure that Valerie is receiving financial support
and living allowance from the government as she is still studying and can't provide for
herself due to the low-income, as the local government and the NHS will be assessed
as well, in order to see that practitioners are offering service users like Valerie who
suffer from mental issues the right treatment by meeting their needs and taking into
account all the mental health organisations and resources out there and bring them
to the local area where Valerie lives at, as this will be beneficial to meet her needs.
Furthermore the HWB will work alongside the CCGs to assess public health by making
sure that Valerie is provided with the right services such as gyms, parks, therapeutic
groups etc. What the CCG organisation does is an ongoing process of guaranteeing
that the population receives a good integrating health care. As well as the Health and
Wellbeing Boards, CCGs was created to follow The Health and Social Care Act 2012,
which is a legislation that firstly introduced legal duties about health inequalities,

which includes specific duties from health bodies which are department of public
health of England, clinical commissioning group, and the NHS. Overall, the Health and
Wellbeing Boards not only work in Valerie's best interests by addressing her needs,
but they also ensure that the local authority and the area in which she lives are acting
equally by delivering the appropriate care to address service user needs and
enhancing population health with the help of the clinical commissioning group as
well as other boards.

Billy
In Billy’s case, I chose to involve the Education, Health and Care Plan (EHC), which is a
legal document that describes a child or young person's special education, health and
social care needs, as well as the extra help that will be provided to meet those needs
and how the help will be able to support the child or young person in doing what
they want in life, and is only given to those aged up to 25. This plan is being assessed
by the local authority in order to see if the certain individual has a health condition or
has been living certain circumstances that enable them to be in school, college or uni.
As in Billy's case his bad circumstances lead him to be homeless, after an argument
with his mom’s partner that seems violent towards him, this situation altered his
health condition as he had an asthma attack. Therefore, in this case the local
authority where Billy lives will be referring him to a children service department
which is responsible to protect and support vulnerable young people and children by
providing them or their family extra help or protecting them from any risk of harm or
abuse. In Billy's case, the children's services will be looking at the environment he
was exposed to, this matter will then be reported to the local authority who will be
able to find him an accommodation to stay at once he will be discharged from the
hospital. As Billy is still 18 years old his capable to get help from the education health
and care plan, as his still young this agency will be able to get him to a good college
or university that will support his needs by providing him all the sercose needed to
get his life back in track and overcome all those challenges, such as him having visual
impairment and asthma. Overall, this education plan is mainly needed for those who
are in education (EHC) will work alongside the local authority to provide a better life
and future by meeting all Billy educational, social, financial and health needs.

Aisha
For our last service user Aisha H. I choose to involve the role of the organisation
responsible for commissioning social care services. Commissioning involves deciding
which services are needed for the diverse local population by ensuring that they are
provided, this agency work as well alongside the Clinical Commissioning Group
