Surgical infections have led to the reduction in patient safety thereby making hospitals inefficient in discharging their role of providing efficient and effective patient care. The problem, which is unavoidable on the patient’s side, has led to the development of various complications after surgery as well as death of millions of patients across the world. It is therefore imperative to define the problem and its causes, which would help develop possible intervention measures to reduce or eliminate incidences of surgical infections. Describe the practice problem in your own words and formulate your PICOT question. The high rate of surgical infections caused by errors and negligence continues the practice problem. The infections range from failure to use Chlorhexidine as outlined in pre-operative procedures, lack of due care and professionalism, common errors and negligence by health care provides. The implementation of measures to reduce the prevalence is a vital step in improving healthcare safety and efficiency. Outline one or two evidence-based solutions you will consider for the trial project. The use of Chlorhexidine as part of the pre-operative procedure is critical in reducing and preventing surgical errors. Chlorhexidine helps reduce incidences of breakdown and irritation, which puts patients at a higher risk of infection. The use of Chlorhexidine requires prior examination to establish whether there is change in strength or regimen specifically among patients with sensitive skin. Another solution is the avoidance of hair removal at surgical sites. Hair removal increases the chances of having cuts and irritations that exposes patients to infections. The alternative to hair removal is the use of clippers if possible
