This week's graded topic relates to the following Course Outcome (CO). CO4: Integrates clinical nursing judgment using effective communication strategies with patients, colleagues, and other healthcare providers. (PO#4) Change is part of our daily healthcare environment. Clinical nursing judgements based on quality and safety, as well as protocols and standards, reflect our patient-centered care values. • What steps could you take to develop more resilience in your practice setting? • How might you communicate a change in practice to patients and nursing peers? A recent change in practice at our hospital was the use of tele sitter cameras for patient safety. As with all health care facilities we experience the challenge of keeping our patients safe. This was a project that was researched and initiated because of our fall rate, high incidence of overtime being paid for support staff to sit in the patients rooms on safety watches and there fore pulling them off of the floors where they could be delivering care to a whole section of patients. Staff felt that there was an impact in quality of care due to the decrease in staff on the floors. Nurses who represented their floors at our Staffing retention advisory council were complaining about their concerns about quality of patient care with the decreased amount of support staff working on the floors because of the high number of safety watches throughout the hospital and on their units in particular. One of the nurses on our council had been employed at a hospital in the Midwest that used cameras for patient safety and suggested us researching this for our hospital. We conducted research and we located hospitals that were using cameras for patient safety and arranged field trips including representatives from Quality and Risk Management along with staff RNs and leadership. We created a business plan and presentation to our finance officer and a pilot of 14 mobile tele sitter cameras was approved. After training our float team to be the observers and rounders for the tele sitter cameras and educating our entire hospital staff about this new technology for patient safety we were ready to embark on the process of implementing the cameras and rolling out, our project. We were optimistic for success. “While all changes do not lead to improvement, all improvement requires change” (, 2016). As RNs we are expected to be "change agents and drive forward new processes, policies and technology to improve the care we provide to our patients and families" (Thomas, Seifert, Joyner, 2016). Our motivation and excitement about the tele sitter cameras were clearly just for our team. Our innovative practice was met with great resistance. Nurses were very cautious about trusting a person watching their patients on camera for safety vs a one to one and the support staff were very resistant to collaborate and work with our support staff float
