Manage Discussion Entry Caring in contemporary nursing can be defined as a relationship with patient characterized by empathy, attentiveness and experience and sensitivity approach. Through this method of caring and active communication, providing information which reduces stress and anxiety can also help in breaking down the stuff. In this model, we require the nurses to have their availability, reliability, emotional and physical support along with their qualification qualities to assist the patient care. In the modern day, it is more of an active conversation where the patient is free to talk to the nurse and have a mutual cooperation. This also gives rise to many other prospects such as good recovery and efficient caring of a patient, the diet follow up and keeps the health bills in check and prescriptions on par with the situation. Now if you consider, here is also another way of dealing with this system which as given rise to a family nursing practitioner(FNP). Some say it is rather like more of a mini version of the family doctor. The FNPs do their normal job such as diagnose illness, prescribe medication therapy, routine check- ups, order lab tests and along with that assist in the minor surgical process and focus on disease prevention. This approach is more centered towards the patient. The nurse keeping in mind the ethical principles of the profession shall apply them to the best of their merit and try to develop more of a friend rapport with the patient. In previous times, we had more of a doctor to a patient relationship which has been slowly transforming into more of a friendly relationship and mentoring the patient with ease. One of the local hospitals ten years ago realized a nurse needed to have a sense of well-being before she could take care of others. This hospital staged a campaign to make sure the nurses took care of each other as well as the patients. Many other hospitals have embraced this concept. You mentioned that the dynamic of patient to nurse relationships has shifted to a friendly/mentorship approach with a focus on co-managing care. I believe this has shifted due to advancements in technology such as the internet. The internet allows patients to easily access health information. Patients and families can educate themselves on diagnoses and treatment options, and health information is no longer only accessible to health professionals. A study conducted by Kim, Silver and Gustafson showed patient internet information seeking affects the physician patient relationship (2017). This can be applied to patient interactions with other healthcare professions such as nursing. Research concluded that due to the availability of information on the internet, patients are more informed and more involved in healthcare decision making and therefore traditional patient to healthcare provider relationship models must shift to meet the changing needs of the modern-day patient. I like that you incorporated the importance of building a nurse patient relationship with having mutual cooperation between the nurse and patient. As we know a nurse- patient relationship that is therapeutic is defined as a relationship that's built on mutual trust and respect, the nurturing of faith and hope, being sensitive to self and others, and assisting with the gratification of not only your patient's physical needs but also their, emotional, and spiritual needs with knowledge and skills learned. It is important to build up a good relationship with all patients this can all be accomplished through building trust, respect and good communication skills are all essential but the counselling relationship has to go that bit further because it tends to deal with deeper issues An article that I found online states that "Providing care in a manner that enables your patient to be an equal partner in achieving wellness. When considering the nurse’s duty to protect and support patients, caring can be conceptualized as the careful treatment of the patient while working together and coordinating with other healthcare professionals (Drahošová & Jarošová, 2016). Some perceive caring to be the very essence of nursing. The nursing profession is a dynamic platform so defining its essence proves challenging. When one seeks to fully describe caring as it relates to nursing, they will find the definition and concept of caring to be quite ambiguous. I am pursing a Master’s of Science in Nursing, Nurse Executive track and plan to continue my education to earn a certificate in Nurse Education. I deeply understand that caring coupled with an evidence-based holistic focus should remain the foundational essence of the nursing profession. I find value in Chamberlain’s holistic educational model and the institution’s philosophy to take “extraordinary” care of students which will lead to extraordinary care of patients, families, and communities (Chamberlain College of Nursing, 2016). I too believe caring for one another as nursing professionals is crucial to providing the best care possible for patients and families. As a nurse leader/executive I will uphold what it means to care by contributing to the transformational growth and development of nurses. I plan to apply this concept of caring to professional practice by establishing an environment of preceptorship and mentorship among professional staff. Formal mentorship programs can help address issues within the profession such as poor retention rates, which in turn results in poor patient care (Jones, 2017). Facilitating mentorship prepares the next generation of leaders, and fosters achievement and fulfillment in junior staff (McCloughen, O’Brien, & Jackson, 2013). Orem believed that to be healthy an individual must treat her body in all realms - physciological, physical, mental, and spiritual. She believed it was an individual's responsibility to take care of herself, and the nurse would take care of the sick patient. There is a major gap between the role of the doctor and the nurse when caring for a patient in the hospital. The nurse is expected to treat the patient holistically, developing a care plan that affects every body system. As nurses we recognize that if one system is abnormal, many other body systems will be affected. A doctor is a specialist and only treats one body system. My nursing model theory in Ventura California at Ventura College for my ADN adopted Dorothy Orem's Theory and self care deficit theory. To this day, I find myself quickly assessing patients organizing my care on the self care deficits of knowledge for example and use this to develop an ongoing plan of care in my daily assignments. I work at an Army facility and the soldiers come back with many self care deficits from battle and the care concept is extremely powerful to engage them both physically and emotionally to allow them to increase self esteem and awareness as a soldier and father, and spouse. We as nurses "care" as an adjective in this sense as well as a noun. As a nurse, we are expected to model the correct behavior. A good example of this are the rules and regulations governing the dress code for a nurse. A nurse who looks unkept with dirty fingernails and hair does not set a good example for her patients, nor does she portray a confident nurse. Although it is always acceptable for a nurse to wear white, tight white levi jeans does not look professional nor do long fake fingernails. Being in the military, you have a strict dress code but many institutions do not enforce this code. Dr. E. I have noticed that little research has been done on the application of nursing theories to nurse practitioners. As a fellow future FNP, I thought you might be interested in King's Goal Attainment Theory and how it may apply to your future practice. King focuses on the interactions between the nurse and patient during which time they develop a trusting relationship, mutually set goals and both agree on a way to meet the goals (De Leon-Demare, MacDonald, Gregory, Katz, & Halas, 2015). De Leon-Demare, MacDonald, Gregory, Katz, and Halas (2015) conducted research to determine if and how King’s theory integrates with NP practice and found the personal way NPs interact help to strengthen the relationship between the NP and the patient and demonstrated the theory demonstrates respect as a key component of the relationship that sets the tone for successful “information exchange, goal setting, and patient-centered care.” The joint goal setting helps patients feel cared for, listened to, and valued while accomplishing the ultimate goal - participating in improving their health. Physicians often focus on disease process and physical body systems while neglecting to treat the whole person. The nurse, like the physician, addresses clinical issues related to body systems and disease process but is also responsible for providing compassionate care. This distinguishes nursing as a profession. She holds the responsibility within the healthcare team to advocate for the rights and overall care of individuals to promote ideal health. The concept of holistic nursing centers on patient advocacy and is critical to the healing experience of the patient. The nurse influences how the doctor treats the patient by advocating for a multifaceted approach that recognizes the patient as an individual with interconnected needs. She has a responsibility to communicate and collaborate with physician staff and serve as a leader within the interdisciplinary team when formulating care plans and interventions that involve whole person care. As an advocate the nurse mentors patients on self-care and self-advocacy.
