Statement of Truth: Contents
Claim Form (and PoC if served later) must be Claim form must:
verified by SoT. 1. Contain a concise statement of the nature of the claim;
2. Specify the remedy sought by C;
• Must be signed by claimant/legal rep 3. Contain other matters:
• Must specify date of issue of claim form (if a. C’s residential/business address
separate doc from Claim Form) b. If D is individual, D’s residential/business address
c. Postcode
Claim form verified by SoT can be used a d. Title of proceedings
evidence by C at any hearing (except for trial)
e. Full name of both parties/ Trading name/
CF may be struck out by Court if not verified by partnership name
