Healthcare Effectiveness Data and Information Set (HEDIS) has 6 domains of care:

Effectiveness of Care, Access/Availability of Care, Experience of Care, Utilization and Risk

Adjusted Utilization, Health Plan Descriptive Information, and Measures Collected Using

Electronic Clinical Data Systems. According to NCQA website, “HEDIS is a performance

measurement tool used by millions of health insurance plans (HEDIS, 2020).” For this paper

Effectiveness of Care will be the chosen domain and Controlling High Blood Pressure is the

HEDIS performance measure. The purpose for this paper is to provide information on controlling

high blood pressure by providing three interventions, how it improves patient outcomes, cost

savings, and patient ratings. Hypertension is a leading cause of cardiovascular disease in the

United States more than any other modifiable risk factor. (Muntner, Hardy, Fine, Jaeger,

Wozniak, Levitan, & Colantonio, 2020) As an APN cardiovascular disease will be the most seen

in practice than any other disease and as APNs treating these patients is very important for their

health.

Patient Interventions

Prevention One: Weight Management

Hypertension can develop through modifiable risk factors and non-modifiable risk

factors. Obesity is one of the leading causes of hypertension and when weight is managed

properly hypertension can be controlled. In certain communities’ obesity and hypertension are

rapidly increasing and is seen as a health concern that can possibly turn into a global epidemic.

Studies show that obesity leads to increase heart rate, vasoconstriction, and increase blood

volume that affects the heart which increases vascular disease. (Ardahan & Konal) The best way

to monitor and track the patient’s weight is to have them weigh themselves daily at the same
