Why do I need someone to help me now? I am not that bad off,” what other health care team member and/or support services should be recommended? You can have someone from social services come and get him someone to talk to because he has been so used to being independent it will be a change that he is no longer as independent. PCC What risk factors does Vernon Russell have (modifiable and nonmodifiable) that indicate the risk for reoccurrence of a stroke? He has a history of hypertension, coronary artery disease, and diabetes mellitus type 2. He has smoked over a pack per day for the past 35 years and does not exercise. PCC What patient education should be initiated due to the identified modifiable risk factors? Smoking a pack daily is something that should be changed. Concluding Questions Describe how you would apply the knowledge and skills that you acquired in Vernon Russell’s case to an actual patient care situation. I would appl;y the skills by doing the same steps I took here by using Sbar, also by including teaching so that he understands what can happen and why it Is happening. What opportunities for improvement should you address?
