Objective: Assess ability to take comprehensive antenatal history. Equipment:
ANC card, pen, chair Steps:
1. Wash hands and introduce self to client.
2. Confirm identity and ensure privacy.
3. Ask obstetric history: gravida, para, previous deliveries, complications.
4. Record LMP and calculate EDD.
5. Take medical history (hypertension, diabetes, infections).
6. Take surgical history.
7. Ask about family and social history.
8. Ask current pregnancy symptoms.
9. Summarize and confirm findings.
