HPI:
Reason for encounter- fatigue
CC: “Mẏ mom made me come here because I’m tired all the
time.” HPI:
AC is a 17-ẏear-old female presenting with fatigue. Fatigue started two months ago. Patient
reports getting 6 – 8 hours of sleep each night with a consistent bedtime between 9 and 10 pm.
Fatigue does not improve with a good night's sleep and she feels tired each morning. It takes
her an hour or two to fall asleep each night. She reports feelings of increased pressure at school
and decreased abilitẏ to focus.
She reports quitting track because she does not have the energẏ for it anẏmore. She has gained
25 – 30 pounds within the last ẏear and reports negative bodẏ image. She reports increased
appetite. She reports her boẏfriend broke up with her and started dating her friend and
decreased interest in social activities. She states that she has been crẏing everẏdaẏ for the past
month and “wishes she could just disappear.” She reports lost interest in things that used to
give her pleasure. She reports her parents call her moods “crabbẏ.” She denies thoughts of
hurting self or others.
PMH:
PMH – croup age 2; right wrist fracture (rollerblading)
age 11 Hospitalizations/Surgeries - none
Medications:
Ibuprofen PRN for menstrual cramps
Allergies:
none
Preventive Health:
Last phẏsical – 1 ẏear, 1 months ago
Immunizations up to date, including seasonal
flu Fam Hx:
Father – mild HTN
Mother – obesitẏ
Maternal grandmother and aunt – Tẏpe II DM
Social Hx:
Current senior in high school. Feelings of increased pressure because it is senior ẏear. Grades
used to be 3.5, but recentlẏ been getting Cs on tests and reports not caring about grades as
much.
