In this assignment, I will be explaining two examples of multi-isciplinary working in health
service provision. The two examples I will be using are Tom an- Latoya.
Mult--issiplinaar an- mult-agensr woakingn This is when -iferent professionals work
together. For example, social worker working with a -istrict nurse, police working with
social workers or NHS Trust working with NHS foun-aton trusts. It involves -rawing
appropriately from multple -isciplines to explore problems an- reach solutons base- on a
new un-erstan-ing of complex situatons. Furthermore, multi-isciplinary working an-
multiagency working involves appropriately utlising knowle-ge, skills an- best practce
from multple -isciplines an- across service provi-er boun-aries. For example, health an-
social care or voluntary an- private sector provi-ers to re-efne, rescope an- reframe
health an- social care -elivery issues an- reach solutons base- on an improve- collectve
un-erstan-ing of complex patent nee-/s.

Tomn
In Tom's scenario, there are examples of multi-isciplinary working. The frst example is
Tom's carer Jason who is manage- by a Care Agency an- works closely with Tom an- Tina.
His responsibilites may be provi-ing personal care for Tom an- -iscussing the care that Tom
nee-s to receive with Tina. Tina is relate- to Tom so any -ecisions regar-ing Tom will have
to be -iscusse- with her frst. Also, Jason may report Tom's progress back to Tina an- he
woul- tell her how Tom's -ay went while she was at work. Jason may also work with a
-ementa specialist nurse/-istrict nurse. The nurse woul- a-vise Jason on how to care for
Tom so that his -ementa can get beter. Another example is a social worker who may be
calle- to work with Tom an- investgate any safeguar-ing concerns an- allow him to
functon in -ay to -ay life. The social worker woul- work with all professionals involve-.
Their role woul- be to ensure that Tom is not put at risk of any harm or abuse. Furthermore,
the police coul- work closely with the social worker an- have an 'at harm system'. The last
example is, an a-miral nurse who may be supporte- by NHS Foun-aton Trust an- work
with the GP. She woul- be responsible for supportng Tom an- his family with Tom's
-ementa. She may also work with a social worker to share informaton on Tom's -ementa.

Latoran
In Latoya's scenario, there are examples of multi-isciplinary working. The frst example is
the social worker who woul- work with the Foster carer an- specialist organisatons for
unaccompanie- minors or asylum seekers, such as Coram. Coram is a charity that provi-es
practcal an- emotonal support for chil-ren an- young people who are vulnerable. Another
example is the foster carer who woul- carry out care of Latoya -ayitoi-ay, an- woul-
report to the social worker any concerns or changes. An example of a concern woul- be if
Latoya starte- having mental health problems -ue to the stress from being alone in a
foreign country an- having a baby at a young age. Furthermore, mi-wives woul- work with
