chest pain.
• Onset: Began 3 weeks ago.
• Duration: Each episode lasts 5–10 minutes.
• Character: Pressure-like, squeezing, non-pleuritic.
• Location: Substernal, radiates to left arm/jaw.
• Aggravating Factors: Physical exertion (stairs, heavy lifting).
• Relieving Factors: Rest; resolves within minutes.
• Associated Symptoms: Shortness of breath, mild nausea, sweating.
• Severity: 6–7/10 during episodes.
III. Past Medical History
• Hypertension (poor adherence to medications).
• Hyperlipidemia.
• No history of diabetes or prior heart attack.
