Organizational Plan
1. Establish a governance structure. How will the new
organization/affiliation be structured?
Twin Pines Medical Center (TPMC) is looking to affiliate with another healthcare
organization to provide the specialty care they lack. TPMC needs to find an
affiliation to provide the care that is missing in the Pine Parish area. The Mayo
Clinic would be an ideal fit for TPMC. The TPMC could join the Mayo Clinic
Network, which has partnered with clinics worldwide. This allows TPMC to remain
independent but share the Mayo Clinic's expertise.
2. Strategy. Describe the overarching vision and mission of the affiliation.
Mission: To collaborate with leading healthcare organizations to bring affordable
quality healthcare to the Twin Pine Medical Center community.
Vision: To provide personalized specialty care to the community through a
multidisciplinary approach to improve patients' quality of life.
3. Sources and resources. How will people and financial resources be
allocated to accomplish these goals?
It would require effort from both TPMC and Mayo Clinic employees to meet the
goal. A referral method would need to be created to schedule a TPMC patient
with Mayo physicians for specialty care. Since the clinics are not within travel
time, appointments would need to be through telemedicine. Case management
would be required to ensure continuity of care. In addition, IT would need to
develop and maintain a secure network. These roles can be filled by existing
staff or by hiring new personnel to fill the positions.

TPMC's primary goal is to offer specialized diabetes care before it progresses to
the patients going to the ED. The diabetic patients who visit the TPMC ED are
between 22%- 24.7% in 2021 and 2022. Success can be measured by the
decrease in diabetic patients presenting to the ED. Since much of the Pine
Parish population is uninsured or underinsured, resulting in deductions from
revenue, this can be improved by preventive care in the diabetic population.

and case management, the ED visits would decrease, resulting in increased
revenue.

mitigate potential risk points, such as miscommunication and negativity

The executive board members and department heads of participating TPMC and
Mayo Clinic clinics would conduct meetings to discuss potential obstacles that
may arise through the affiliation. They need to ensure the best interests of the
patients and employees are addressed.

be involved in the affiliation activities.
The best management system would be utilizing the Mayo Clinic Network, a
clinical network with shared expertise and resources. There is an affiliation
between TPMC and the Mayo Clinic, and both remain independent. Not merging
allows TPMC to retain all its employees from the highest to the lowest level.
