Antiemetic/ 5-HT3 antagonists

PURPOSE OF MEDICATION
Expected Pharmacological Action Therapeutic Use
Blocks the effects of serotonin at 5HT3 Decreases incidence and severity of n+v
recptor sites located in the vagus nerve following chemotherapy, radiation, or surgery
terminals and the chemoreceptor trigger
zone in the CNS
Complications Medication Administration
CV: torsades de points (tachycardia that starts in ventricals, can cause cardia arrest), IV
QT interval prolongation O: rapid P: 15-30mins D: 4-8hrs
GI: diarrhea, constipation, abdominal pain, dry mout, increased liver enzymes
Neuro: headache, Serotonin syndrome (increase serotonin, deadly), dizziness, Halflife: A: 3.5-5.5hr; C(5m-12y):2.9 hr
drowsiness, weakness,
Derm: steven johnson syndrome (flu like symptoms, painful rash, skin dies and peels) IVP: concentration: 2mg/mL
Rate: at least 30sec; perf. over 2-5mins
IVPB:Concentration: 1mg/mL
Rate: over 15mins
Contraindications/Precautions
CI: hypersensitivity, conginital long QT syndrome, current
use in apomorphine
Precaution: hepatic impairment, abdominal surgery, Nursing Interventions
Phenylketonuria (amino acid and phenylalanine build up) -Assess for n+v,abdominal distension,
bowel nsounds
-Assess for rash (SJS)
-Assess s/s of serotonin syndrome
(agitation,
hallucination,tachycardia,tremor)
Interactions -Assess for extrapyrmidal effects
Apomorphine: high risk of sever Hypotension and loss of conciouness (involuntary movements facial
Carbamazepine, phenytoin, and rifampin may decrease levels grimacing, shuffed walking)
SSRIs, MAOIs, tricyclic antidepressants increase Seroronin syndrome
Can cause transient increase in serum bilirubin, AST, and ALT levels
Client Education

-Notify PCP immediatly if

Prevention of n+v postop, post chemo, post radiation movements of eye,face, limbs
