The Bank of Lisbon Building Fire: Revealing Safety Flaws and Urging Improved Fire Protection The Bank of Lisbon Building Fire occurred on September 5, 2018, in Johannesburg, South Africa. Several government agencies, notably the Departments of Health, Human Settlements, and Cooperative Governance and Traditional Affairs, were based in the city centre building. The building's 23rd floor was where the fire first started, and it swiftly spread to other stories, devouring them all. To put out the fire and get the building out of the way, firefighters were called to the scene. Tragically, while tackling the fire, three firefighters passed away. The incident exposed several safety issues and weaknesses in the Bank of Lisbon building. It was discovered that the structure lacked sufficient fire detection, suppression, and alarm systems, including sprinklers, fire extinguishers, and fire alarms. Inadequate evacuation protocols, clogged stairwells, and defective emergency exits were also mentioned. The Bank of Lisbon building fire revealed violations of safety standards and sparked worries about the general level of fire safety in governmental structures. The tragedy sparked investigations into the safety norms and procedures used in several South African government owned buildings. Exactly what started the 2018 Bank of Lisbon Building Fire in Johannesburg was determined to be an electrical fault. The electrical supply room on one of the top storeys of the structure is where, according to sources, the fire first broke out. It is important to keep in mind that electrical problems can occur for a variety of reasons, such as defective wiring, overloaded circuits, damaged equipment, and inadequate maintenance. The electrical fault's presence, along with the building's defects and weaknesses in terms of safety, aided in the fire's rapid spread and ferocity. Early identification and containment of the fire were hampered by a lack of sufficient fire prevention and suppression systems, such as fire alarms, sprinklers, and fire extinguishers. Insufficient emergency exits and blocked stairwells also impeded the evacuation process, making it challenging for occupants and firefighters to escape the building safely. The Bank of Lisbon Building Fire served as a sobering reminder of the need for adequate electrical upkeep, compliance with safety laws, and the requirement for extensive fire protection systems in commercial buildings. The importance of routine inspections, prompt repairs, and adherence to electrical safety regulations was underlined to avoid such tragedies and reduce the hazards associated with fires. Following the incident, emphasis was placed on enhancing safety procedures, guaranteeing adherence to fire safety laws, and enacting stronger building codes for government buildings. The tragedy served as a reminder to the authorities that the protection of occupants and first responders in public buildings should be prioritised, and that strict fire safety standards must be implemented. Question 1 1. What was the primary cause of the Bank of Lisbon Building Fire? Question 2 2. Explain how you think this could have contributed to the rapid spread of the fire. Identify and explain any three factors that leads to the spread of fires in buildings using the information from the case study. Question 3 3. Identify at least four safety violations in the building that worsened the impact of the fire. Question 4 4. How did the lack of fire suppression and detection systems affect the response to the fire? Question 5 5. What challenges did firefighters and occupants face during evacuation? Question 6 6. How could these challenges have been prevented or addressed? Question 7 7. We often read of many instances where fires have erupted in buildings and the occupants have to be evacuated as quickly as possible in an effort to prevent injury and loss of life via means of egress. Explain the means of egress when it is necessary to move people out of a building as quickly and safely as possible. Question 8 8. Provide recommendations on fire prevention strategies that should be taken into account to prevent office fires.

Safety Management IIA

Question 1:
What was the primary cause of the Bank of Lisbon Building Fire?

The main cause of the Bank of Lisbon Building Fire was an electrical fault. The fire
started in the electrical supply room on one of the higher floors of the building. Problems
like faulty wiring, overloaded circuits, damaged equipment, and poor maintenance likely
caused the electrical fault. Unfortunately, once the fire started, it was very hard to stop
because the building had many safety problems.

Question 2:
Explain how you think this could have contributed to the rapid spread of the fire.
Identify and explain any three factors that led to the spread of fires in buildings
using the information from the case study.

The electrical fault contributed a lot to how fast the fire spread. When a fire starts in an
electrical room, it can easily catch other wires, cables, and materials that burn quickly.
Without working fire alarms and sprinklers, no one noticed the fire early enough to stop
it.

Three factors that led to the fire spreading so fast were:
