changes, possibly depression or anxiety.
• History of Present Illness (HPI):
o Onset and duration of mood
symptoms.
, o Recent stressors (work, relationships,
health, financial).
o Any history of trauma or significant
life events.
o Sleep disturbances, appetite changes,
energy levels.
o Suicidal ideation or self-harm
thoughts.
• Past Medical History (PMH):
o Previous mood disorders (depression,
bipolar disorder, anxiety).
o History of psychiatric
hospitalizations or therapy.
o Chronic medical conditions
(hypothyroidism, chronic pain, etc.).
• Medications:
o Current prescriptions, over-the-
counter meds, supplements.
o History of antidepressant or
anxiolytic use.
