Six steps administering any medication ANSWERS Must have local protocol or
order from medical control and repeat order back to physician (echo technique).
- Always ask for clarification if necessary
1. Right patient
- NEVER give a prescription not in her name
2. Right medication
- Check for contraindications
3. Right dose
- How many milligrams per tablet, etc.
4. Right route
5. Right time
- Expiration date and condition (don't use if date smudged)
- Reassess at least every 5 min if condition changes (especially BP and HR)
- If ordered to repeat the medication, make sure enough time has passed/haven't
reached max dose
6. Right documentation
- Time you gave the medication
- Name, dose, and route of administration
- Change to patient condition and side effects
Reassess after administration. Look for side effects and document.
