CN 1 - olfactory - nostrils / identify oder
CN II - optic - vision with Snellen 4 ft & 20 ft
CN II, III - 2 - optic / 3 - occulomotor -- pupillary reactions
CN III, IV, VI - extraocular movements
CN IV - trochlear (EOM)
CN VI - Abducens (EOM)
CN V - Trigeminal - test jaw and dull / sharp on face (facial sensation)
CN VII - Facial movements - forehead, eyebrows, smile/clench, puff cheeks, whistle
CN VIII - acoustic - whisper test
CN IX 9 - Glossopharyngeal - say ahh, inspect soft palate, check gag reflex with sip of
water
CN X - Vagus - gag reflex, check uvula (X and IX go together)
CN IX 11 - Spinal Accessory - shoulder shrug and turn head against resistance
CN XII - Hypoglossal - tongue strength (up, down, side to side) push against cheek
cerebellar function tests - gait, walk back heel to toe, romberg test
coordination tests - rapid rhythmic, finger to nose, heel to shin, repid alternating
stereognosis - identify object in hand (pen)
graphesthesia - draw number in hand, identify
heaves / lifts - sustained
