Inleiding

,  Uit veel onderzoeken is gebleken dat er een relatie is tussen tegenslagen in de jeugd
(childhood adversities, CA) en psychiatrische stoornissen
o Eerdere onderzoeken focusten zich meer op één enkele CA, maar er is
gebleken dat CA’s zich vaak clusteren
o Daardoor is het belangrijk om naar meerdere CA’s te kijken om overschatting
van die ene CA te voorkomen
 In dit onderzoek wordt verder gebouwd op eerdere bevindingen van NCS door
tegenslagen in de jeugd te analyseren in de NCS-R (National Comorbidity Survey
Replication)

Methode
 Het onderzoek bestaat uit twee delen
o Deel 1: diagnostische beoordeling
o Deel 2: participanten met een of meerdere stoornissen werden geselecteerd
 Stoornissen die mee worden genomen in het onderzoek:
o Stemmingsstoornissen: depressieve stoornis, dysthyme stoornis, bipolaire
stoornis 1, bipolaire stoornis 2, subthreshold bipolaire stoornis
o Angststoornissen: gegeneraliseerde angststoornis, paniekstoornis,
agorafobie, specifieke fobie, PTSS
o Gedragsstoornissen: ADHD, ODD, CD
o Verslavingen: drugsmisbruik, alcoholmisbruik, drugsafhankelijkheid en alcohol
afhankelijkheid
 Childhood adversities
o Interpersonal loss: overlijden van ouder(s), scheiden van ouders, gescheiden
worden van ouders/verzorgers
o Parental maladjustment: ziekte van ouders, verslaving van ouders,
criminaliteit van ouders, geweld van ouders
o Maltreatment: fysieke mishandeling, seksuele mishandeling, verwaarlozing
o Andere CA’s: levensbedreigende ziektes bij kind, tegenslagen op financieel
gebied

Resultaten
 Meer dan ½ van de deelnemers kreeg te maken met minstens één CA
o Meest voorkomend: scheiden van ouders, huiselijk geweld, financiële
problemen en ziekte van ouders
o De deelnemers hadden gemiddeld last van 3,2 CA’s
 Uit resultaten bleek dat CA’s sterk samenhangen met het ontstaan van de 4 groepen
stoornissen
o Toch zijn er wel verschillen gevonden in CA’s
 Relatie tussen criminaliteit van ouders en verslaving minder consistent
dan de relatie tussen verslaving van ouders en verslaving
o Ook verschillen tussen de verschillende stoornissen
 Er is wel een relatie tussen financiële problemen/ziektes en
angststoornissen, maar deze relatie is er niet bij stemmingsstoornissen

Introductie

,  Kindermishandeling is een groot probleem in verschillende gebieden
o 1/5 vrouwen en 1/13 mannen is seksueel misbruikt tijdens jeugd
o ¼ volwassenen rapporteert fysiek mishandeld te zijn
 Kindermishandeling kan leiden tot het ontwikkelen van depressie en angststoornissen
o Het draagt bij tot het ontstaan van psychopathologie d.m.v. neurobiologische
mechanismen
 Stress systemen, structurele verschillen in hersenen, verschillen in
functioneren van de hersenen, genetica en epi genetica van
veerkracht en kwetsbaarheid
o Kindermishandeling is een bedreiging voor optimale ontwikkeling van
affectieve processen, gehechtheidsrelaties, ‘self-system’ processen, relaties
met peers en aanpassingen op school
 Depressie & angststoornissen zijn de meest voorkomende mentale ziekten
o Onderzoek: er is een relatie tussen kindermishandeling en psychotische
stoornissen als depressie, bipolaire stoornissen, angststoornissen,
verslavingen en suïcidale gedachten
 Er zijn lange termijn gevolgen van kindermishandeling gevonden
o De kans op depressie en angststoornissen is groter
o Volwassenen die mishandeld zijn rapporteren ook vaker PTSS, cognitieve
verstoringen, emotioneel leed, eetverslavingen, slaapstoornissen,
verslavingen en vermijding
 Eerder onderzoek naar relatie tussen kindermishandeling en depressie/angst: cross-
sectioneel of niet naar geschiedenis van mishandeling gekeken
o Mishandeling vaak gemeten d.m.v. recall
 Leidt vaak tot bias en valse herinneringen
 Mensen die nu een goed leven hebben, vergeten eerder negatieve
dingen uit jeugd
o Dit onderzoek: longitudinale studies
 Wil schattingen geven van door hoeveel de incidentie van
depressie/angst in een bevolking kleiner zou worden bij minder
mishandeling
