Explain what is meant by person-centred
care. (P1)
Person-centred care is where you take into
consideration the service users wants, needs
and goals then make a care plan that caters to
them and that are achievable. This then gets
put into practise, as all care staff are aware of
what the service user wants to achieve and
do.
Person centred care focussing on the
individual and not just the condition that they
have. Person centred care ensures that the
person has a choice in how their care and
support is delivered.


Summarise the 6 C’s of care. (P2)
1. Care is the main thing that nurses do
and the care they provide helps to
improve health of the whole community.
The patients that receive this care expect

, specific care to their needs and to be
consistent throughout.
2. Compassion is the bond that nurses
build with the service users based on
empathy, respect and dignity. It is
essential to how people perceive their
care.
3. Courage is how nurses make the right
choices throughout their career and do
what is best for their service user. It is up
to all staff to speak out if they have
concerns and have the strength to adapt
to each new environment.
4. Communication is important to a
nurse’s success as it helps them to build
caring relationships with their patients
and work well as a team. Listening is
important for good communication, so the
service user knows what is happening and
feel that they are being heard.
