LOC Correct Answer - Alert- responsive and fully able to respond by opening eyes and attending to a normal tone of voice and speech; answers questions spontaneously and appropriately Lethargy- client is able to open eyes and respond but is drowsy and falls asleep easily Stupor- Requires vigorous or painful stimuli (pinching tendon or rubbing sternum) to elicit a brief response; may not be able to respond verbally Coma- No response can be achieved from repeated painful stimuli; involves abnormal posturing such as Decorticate rigidity- flexion and internal rotation of upper extremity joints and legs Decerebrate rigidity- neck and elbow extension, wrist and fingers flexion. Cognitive and intellectual abilities Correct Answer - Orientation to person, place, and time. Check clients memory: 1) immediate- repeat series of numbers or list of objects, 2) recent- ask client to recall recent events, such as visitors from the day or purpose of admission, and 3) remote- ask client to state a fact from his past that is verifiable (birth certificate or mothers maiden name). Whats their perception of their illness- whats their knowledge of their current condition. Clients ability to calculate- counting backwards from 100 in sevens. Check clients judgement based on his answer to a hypothetical question- What would you do if there were a fire in your room? Answer should be logical. Check clients ability to think abstractly- can he interpret cliches, ability to do so indicates higher level thought process. Collect data about the clients rate and volume of speech as well as quality of language- speech should be articulate and response is meaningful and appropriate. Standardized Screening Tools Correct Answer - Mini Mental State Exam- used to objectively collect data about a clients cognitive status by evaluating 1) orientation to time and place 2) attention span and ability to calculate by counting backwards by seven 3) registration and recalling of objects and 4) language including names of objects, following commands, and ability to write.
