Mijn naam is Lieke Mulder en ik ben eerstejaars verpleegkunde student van de HBO
verpleegkunde doorstroomroute op de Hogeschool Leiden. Dit beroepsproduct behoort tot
CBP 2.4 de rol van de samenwerkingspartner. Dit beroepsproduct is uitgevoerd aan de hand
van een casus, in zorg van de Vierstroom Zorg Thuis (1). In dit beroepsproduct wordt er
aangetoond hoe de auteur van dit beroepsproduct zich ontwikkeld volgens de competenties
van het beroepsprofiel HBO-verpleegkundige, CanMEDS-rol samenwerkingspartner (2). Dit
zal worden vormgegeven door een literatuurstudie naar interprofessioneel samenwerken.
De literatuurstudie zal worden onderbouwd door 3 wetenschappelijke artikelen. Daarnaast
zal de casus worden uitgewerkt in combinatie met de interviews die zijn gehouden met
verschillende disciplines het zorgproces rondom de casus. Deze verschillende disciplines
zullen bestaan uit een collega verpleegkundige, de huisarts en de casemanager dementie.
Verder zal er een conclusie worden getrokken met de bevindingen uit de verschillende
interviews, literatuurstudie en ervaringen uit verpleegkundig oogpunt. Tot slot zullen er een
aantal zorgdoelen worden opgesteld, met als doel om interprofessionele samenwerking te
bevorderen in het zorgproces van de in dit beroepsproduct geldende casus.
