ATI CAPSTONE Community and Leadership Review nurse responsibilities w/ informed consent - - Witness informed consent - ensure provider have client necessary info - ensure pt understood info and is competent to give consent - have pt sign informed consent form - notify HCP if pt has more questions - document reinforcement of info given by HCP - document questions pt had were forwarded to HCP - document use of interpreter advanced directives - - purpose of advanced directives is to communicate a pt's wishes regarding end of life care should the pt become unable to do so - all clients admitted should be asked if they have one - pt who doesn't have one should be given info that outlines their rights r/t healthcare decisions and how to formulate advanced directives
