What chronic disease states are common in patients in skilled nursing facilities? HTN, incontinence, arthritis, heart disease, diabetes, stroke, cancer, and Alzheimer's. Who is part of the SNF/ICF team? Family practice physicians, geriatricians, psychiatrists, nurse practitioners, physician assistants, pharmacists, nurses, medication aids, nursing assistants, physical therapists, occupational therapists, social workers, dieticians, psychologists, administrator, owner, and qualified mental retardation professional. What is the role of a pharmacist on a SNF/ICF team? Review all patient's labs and meds at least once every month. What are the limitations of skilled nursing facilities in rural vs urban areas? There are more beds available in rural homes, but less skilled nursing available. Patients in rural areas also tend to be older and have greater disability than those in urban areas. How is a patient' s stay in a nursing facility paid for? If the patient enters a skilled nursing facility after hospitalization, Medicare Part A will pay for it. If they are moved to a "non-skilled" bed where they just receive custodial care, then payment moves to other payment type (Medicaid, private pay, insurance, etc.).
