Angststoornissen hebben met elkaar gemeen dat er sprake is van overmatige angst en vrees en
bijbehorende gedragsstoornissen. Vrees (fear) betreft de emotionele respons op een
werkelijke of waargenomen naderende dreiging, angst (anxiety) betreft anticiperen op een
dreiging in de toekomst. Angst gaat vaak gepaard met spierspanning en waakzaamheid,
voorzichtigheid en ontwijkend gedrag. Binnen angststoornissen worden paniekaanvallen als
specifiek type angstrespons beschouwd.

Angststoornissen verschillen van elkaar in de aard van de objecten of situaties, en door de
cognities die hiermee gepaard gaan. Door te onderzoeken welke typen situaties worden
vermeden en de inhoud van gedachten te inventariseren, kan onderscheid gemaakt worden.

GENERALIZED ANXIETY DISORDER (GAD)

DSM-V-Criteria (EN)
A. Excessive anxiety and worry (apprehensive expectation, bange voorgevoelens),
occurring more days than not for at least 6 months, about a number of events or
activities (such as work or school performance).
B. The individual finds it difficult to control the worry.
C. The anxiety and worry are associated with three* (or more) of the following six
symptoms (at least some symptoms present more days than not for 6 months);
1. Restlessness or feeling keyed up or on edge (opgedraaid, gespannen).
2. Being easily fatigued.
3. Difficult concentrating or mind going blank.
4. Irritability.
5. Muscle tension.
6. Sleep disturbance (difficulty falling/staying asleep, restless sleep).
D. Anxiety, worry or physical symptoms cause clinically significant distress or
impairment in social, occupational or other important areas of functioning.
E. The disturbance is not attributable to the physiological effects of a substance.
F. The disturbance is not better explained by another mental disorder.

*Only one item is required in children.

Verschillende kenmerken onderscheiden de GAD van niet-pathologische angst. Ten eerste
zijn de zorgen bij GAD overdreven, en belemmeren deze de persoon aanzienlijk in zijn of
haar functioneren. Ten tweede hebben de zorgen bij GAD betrekking op meer
levensgebieden, zijn ze sterker aanwezig en lijden mensen er meer onder, zijn ze langduriger
en duiken ze vaker op zonder aanleiding. Ten derde gaan dagelijkse zorgen veel minder vaak
gepaard met lichamelijke symptomen; mensen met GAD beschrijven dat zij lijden onder hun
constante bezorgdheid, en beperkingen ervaren als gevolg hiervan voor hun functioneren.
