Maine CRMA Certification Test (2023/2024) Rated A Certificate for CRMA lastest Two Years Re-Certification must be done within 30 days of expiried For 8-hours CRMA not not adminstrator any meds after expired Recertified within 30 days of expiration date CRMA should set up a tickler for 90 day for recertification 60 days prior and 30 days after Clinical Checklist to the Instructor Needs to be submited 45 days from the last class Instructor has to submit the checklist & validation form to Dept 60 days from the last day of class The tasks a CRMA is expected & allowed to do as part of the job "Scope of Practice Adminstrator Deceision making, approves changes in systems or procedures, problem solving (Medication Errors, Incident Reports, Mediction Refusals and Safe Practices Directors Coordinates day to day direct services, schedules staff coordinates access to outside services, arranges transportation & services from other agencies, as well as ssigns duties to staff members C.N.A, PSS, DSP Proivde the majority of the daily interactins with the idiviuals receiving care. There duties may include skills teaching, problem solving, motivating, working individuals to achieve goals of choice, develop goal-directed supports, assisting with activites, managing outside activies and offering general support. Facility Staff Catering & Maintenance employees, food services, & maint dept. Nurse Consultants Registered Nurses, who serve on-call medical experts & in advisory capacity when necessary. Scope of Practice CRMA's are allowed to adminster Meds. to persons served by DHHSLicensed Assisted Housing prgroams & other licensed facilities only after they have sucessfully taken a minimal 24 - 45 hour class, passed a written test, and demonstratred mediation adminstration competence to an RN in the classroom or to an RN or CRMA ( with a least 1 yers exp.) in the clincial setting. Med passes with a CRMA only allow the CRMA to work in aLevel III RCF/ home servicing individuals under the Office of Adults with Cognitive & Physical Disablilities & Autism. Legal & Ethical Responsiblities of a CRMA include the following: a..) Adminster meds, in a safe, proper & accurate manner, b.) Record adminstration of meds. The adminstration of meds is not completed until documented. c.) Report incidents. d.) Be aware & knowledgeable of resident's rights. e.) Maintain resident's confidentiality. f.) adminsister drugs according to duly authorized, licensed practitioner's orders. Resources used by the CRMAs Adminstrator, Supervisor Pharmacist, Licensed Nurses, Physician Non-people resources Physician Desk Refernece (PDR), PDR Pocket Guide to Prescrition drugs, Drug handbook, posion control center, websites such as , drug companies, drug inserts from the pharmacy There are 4 links in the infection chain that must be present before infection can be spread 1.) A germ- such as a virus, bacteriu, fungus or parasite. 2) a place for the germ to live & multiply, sucha as person, animal, plant, food, soil or water. 3.) a susceptible host- a person who does not have a resistance (immunity) to the germ. 4.) a way for the germ to enter the host. Different germs may require differnt routes. To break the chain of infection 1.) Good hand washing. 2.) Good housekkeping. 3.) using proctective barriers (gloves, gown, masks, etc) 4.) Immunizations. 5.) store food carefully. 5.) wash fruits & vegtables. 6.) use separate cutting boards. 7.) Use hot soapy water to wash dishes. 8.) Not sharing personal items. Signs of Infection 1.) Inflamed skin (red, hot, swollen or a rash). 2.) Ever or chills. 3.) Pus (green or yellow drainage from a wound) 4.) Nausea or vomiting. 5.) Persistent diarrhea 6.) Sore throat or cough. 7.) painful urination Principles of Standard Precautions 1.) blood, 2.) Semen, 3.) Vomit, 4.) Sliva, 5.) Feces, 6.) Urine All materials instrutments & surfaces should be treated as if they are infected: 1.) thermonmeters, 2.) needles, 3.) incontinet pads & briefs, 4.) bathroom counters and tubs/showers, 5.) dirty water Methods to be used for safeguarding the staff and the residents are as follows: 1.) hand washing, 2.) use a hand soap that cleans & gets rid of germs. 3.) wash hands vefore and after working with a consumer. 4.) wash ahands after removing gloves or any other PPE. 5.) wash hands after handling blood or any other bod fluids. Sharp containers 1.) Be sure sharps container is leak proof , has non-removal lid & is not overfilled. 2.) Never recap used needles. 3.) drop the needles, lancets raors & any other sharp in sharps container. 4.) Never reach into a sharp container. PPE Personal Proctective Equipment What does PPE consist of: 1.) single use gloves- cover the hands when having direct contact with consumer. 2.) Utility gloves- cover for hands when using disinfectants or chemicals for cleaning surfaces. 3.) gown/apron- cover for the skin and clothing when there a chance of being splatter with potentially infective fluid. 4.) Mask- cover to prevent blood or body fluids from splashing inthe face or mouth. 5.) Mouthpiece or plastic airway- prevent the need for direct contact when providing mouth to mouth resusciation. Transmission Based Precautions Contacts, respiratiory, skin and linen, blood borne, enteric Exposure to HIV or HBV exits in every health care setting. In the assisted housing area the incidents of this type of exposure could be by Deep tissue injury, needle stick, & direct contact with blood Procedure for needle stick 1.) report injury immediately to supervisor. 2.) allow wound to bleed freely. 3.) obtain medical evaluation & first aide immediately. 4.) Complete facility incident report. 5.) Remeber - you will be offreda test for HIV & other infectious diseases when you have been exposed. Vital Signs 1. temperature (T). 2.) pulse (P). 3.) Respiration (R). 4.) Blood Pressure (BP). 5.) O2 sats Temperatures 1.) orally, 2.) rectally, 3.) axillary, 4.) otic (tympanic) 5.) skin Temperature Normal is 98.6 F and 37 C Ear scan thermonmeter 1.) Completes in 30 seconds. Temperature is 99.5 F Oral Reading in 3 minutes Normal oral temperature is 98.6 Axillary Reading is 10 minutes and normal aillary temperature is 97.6 F Rectal Placed in the rectum and hold for 3 to 5 mins & temperature is 99.5 F Skin/Transfermal Termperature is 94 F Factors affecting body temperature 1.) age 2.) time of day ( 2 to 3 F between morning & late afternoon. highest temp is usually between 4 to 8 pm & lowest between 4 to 6 am.) 3.) Gender- woman are usually warmer than med because of having more fatty tissue. 4.) Emotions- excitement increase heat and depression decreases temperature. 5.) Exercise - chewing gum can increase temps by 1 F. 6.) Infection - elevated temperature. 7.) food, fluid & smoking - oral temperature should not be taken within 30 mins of intake of food & fluids or smoking. 8.) Septic infection - drops temps. Pulse Radial and Apical Pulses: Radial - utilizaing two fingers over the thumb side of the person's wrist, pressing lightly over the radial artery. Your thumb use never to be used. Normal radial pulse of an adults is 60 to 100 beats pre minute with an average of 80 beats per minute Brachial Pulse- obtianed by placing the index & 2nd figer on the inside of the elbow with the palm of the hand facing up. Carotid Pulse- Placing the index & 2nd finger on either side of the neck running them along side of the outer edge of the trachera windpipe. Aprical Pulse- It is obtained by utilizaing a stethoscope. The diaphragm of the stethoscope is placed approximately mid- nipple line on the left side of the chest. Count for 1 min & between 60 to 100 beats per min is normal. Stethoscope 1.) ear piece, 2.) Bell, 3.) Tubing Respirations Obtained by observing the rise & fall of the chest for 1 full min. Normal rate respiration is 12 to 30 pr minute Blood Pressure A measure of the force of the blood against the walls of the arteries. This tells how hard the heart is working. This is obtained by a blood pressure cuff & stethoscope. To obtain the B/P cuff is placed around the person's bare upper arm approximately one to two inches above the crease of the inner arm. High number & low number Systolic pressure if below 80 there is an issue. and the diastolic pressure Normal blod pressure for adults is 120/80 Oxygen Saturations Meaures the level of oxgen in the blood stream
