1. Anatomie/FysiologieNormale situatie – algemeen:
Groei en ontwikkeling longen:
De longen bestaat uit de bovenste luchtwegen (mond, neus, neusholten, farynx,
larynx), onderste luchtwegen (trachea, hoofdbronchiën, bronchiën bronchioli),
longblaasjes (alveoli), longvliezen (pleurae) en ondersteunend weefsel
Het respiratoire systeem ontstaat in vijf fasen:
1. Embryonale fase (26 dagen-6 weken)
De drie kiembladen (ecdoderm, mesoderm en endoderm) fuseren met elkaar.
Uit de fusie van de endodermale laag ontstaat een primitieve darmbuis (voor-
midden- en einddarm). Ventraal in de voordarm ontstaat een uitstulping, vanuit
hier worden de longen gevormd. De bronchiën vertakken zich drie keer,
leidend tot de primaire vorming van de twee longen.
2. Pseudoglandulaire fase (6-16 weken)
De bronchiale boom ondergaat nog 14 generaties van vertakkingen tot de
vorming van terminale bronchioli. De bronchien worden bedekt met kubisch
epitheel, dit geeft de longen een klierachtig (glandulair) aspect. Gaswisseling
is nog onmogelijk omdat de respiratoire units omgeven worden door slecht
doorbloed weefsel en de diffusieafstand tussen de terminale bronchioli en de
capillaire bloedvaten te groot is.
3. Canaliculaire fase (16-28 weken)
Verlening en verwijding van de bronchioli, waarbij de terminale bronchioli zich
ontwikkelen tot respiratoire en sacculaire bronchioli. Epitheelcellen
differentiëren in type-1 cellen (dunne cellen met weinig cytoplasma die alveoli
gaan bedekken) en type-2 cellen (produceren surfactans). In de laatste weken
van deze fase is gaswisseling mogelijk.
4. Sacculaire fase (28-36 weken)
De respiratoire bronchioli ontwikkelen zich in een laatste vertakking tot
eindstandige terminale sacculi (primitieve alveoli)
5. Alveolaire fase (36 weken – ver in de kindertijd)
Door een proces van septatie en vedunning van het epitheel ontstaan uit de
terminale sacculi mature alveoli

Thorax en pleurae
Thorax bestaat uit drie componenten
- Het hart,
- De longen: links 2 kwabben, rechts 3 kwabben.
- Het mediastinum: grote vaten, zenuwen,
slokdarm en de thymus.
1. Pleura visceralis: ligt om de longen
2. Pleura parietalis: binnenbekleding van de thorax
3. Pleuraholte: de ruimte tussen deze twee
pleurabladen is de pleuraholte.
