Task 2
This report will be based on Fatima who is a 49-year-old mother who lives with her 2 children and
husband. She has
been currently unemployed and spends a lot of time at home. Fatima has been diagnosed with type
2 diabetes and
complains of tiredness, daytime sleepiness, frequently waking up at night to urinate and is
significantly overweight.
There is also a family history of diabetes her mother and sister both suffer with type 2 diabetes. This
report will provide
a justified and personalised treatment plan that reflects the needs, wants and wishes of Fatima
whilst ensuring that his
physical, emotional social and intellectual needs are met ensuring a holistic approach to his
healthcare.
, signs of type 2 diabetes and therefore requires further medical intervention promptly; an early
diagnosis wil
enable the GP to offer the most appropriate healthcare in the early stages of the disorder. The GP
will refer
Fatima to the diabetes specialist nurse who would monitor their blood glucose levels every 2 to 6
months in
order to achieve an appropriate blood glucose target. This would also significantly reduce in long
term vascular
complications of diabetes. Fatima's blood glucose levels (HbA1c) remains high at 79mmol/mol and
has been
prescribes 500mg of metformin to take 3 times a day and 80mg of gliclazide twice a day. It is
important for
health care professionals within this inter disciplinary team providing care for Fatima to note her
contextual
factors given the family history of type 2 diabetes lack of exercise and her weight which would make
her
susceptible to developing type 2 diabetes.
P7A Explain how the plan would improve the health and wellbeing of a selected service user.
This treatment plan would help Fatima improve her health and wellbeing because the treatment
